Social Worker in Rehabilitation Services
Northern Health
Join Northern Health as a Social Worker focusing on rehabilitation. This full-time role emphasizes client-centered care and requires strong assessment and treatment planning skills. As part of the Adult Rehabilitation & Recovery Services team, you will engage in psychosocial support, client advocacy, and treatment plan development. Collaborating with a diverse team, you’ll work towards enhancing client health outcomes through effective strategies and resources while addressing individual needs. Key Responsibilities:
• Complete initial and ongoing client assessments
• Develop individualized care plans collaboratively
• Deliver direct care and support to clients and families
• Educate clients on self-management techniques
• Maintain documentation and administrative duties Requirements:
• Bachelor’s Degree in Social Work from an accredited program
• Minimum of three years' experience in healthcare
• Registered with the BC College of Social Workers
• Valid BC driver’s license required
• Strong organizational and leadership abilities Utilize your social work skills to make a meaningful impact in rehabilitation at Northern Health.
