Key Responsibilities Conduct comprehensive needs assessments for patients and caregivers.
Collaborate on developing and implementing care plans.
Coordinate access to healthcare services and resources.
Facilitate communication among multidisciplinary teams.
Maintain accurate case documentation per health standards.
Requirements Registered with applicable Ontario healthcare college.
Two years of related qualified experience needed.
Bilingual fluency in English and French mandatory.
