Nurse Reach Ontario Long-term Care Project - CARE Centre for IENs

JOB PURPOSE This project will see 100 internationally educated nurses residing in major urban centres across Ontario secure employment and successfully relocate to rural and northern communities where healthcare employers face persistent nursing shortages. The project will provide recruitment, employment matching, relocation supports, community integration services, and retention assistance through a partnership between CARE Centre and Blue Branch Inc.

We are looking for an individual who can work closely, flexibly, and energetically with a small staff team in a principled and pragmatic way, working towards achieving the CARE Centre for Internationally Educated Nurses’ (CARE Centre) mission, vision, and goals. The successful candidate must work with our clients from all regions in Ontario, focus on supporting implementation of the Nurse Reach Ontario Long-Term Care Project while providing individualized support to participating IENs who are interested, prepared, and ready to explore career pathway in Long-Term Care and relocation.

JOB ACCOUNTABILITIES AND RESPONSIBILITIES Key Responsibilities Reporting to the Project Coordinator and working collaboratively with the Nurse Reach Ontario Long-term Care Project, CARE Centre team, and external educational, professional, and community partners to support the successful implementation of the Nurse Reach Ontario Long-Term Care Project. The position combines individualized support to facilitate the recruitment, integration, retention, and professional development of IENs. The Case Manager provides accurate information, effective guidance, and support to enable IENs to meet their individual professional goals and successfully enter the nursing profession in Ontario’s long term care sector using a case management model.

Client Management and Services

Conduct screening, intake, and assessment for clients based on CARE Centre’s Nurse Reach Ontario Long-Term Project enrollment requirements as identified in the Project Manual.

Facilitate completion of the Nursing Readiness Assessments (NRA) and provide timely and appropriate feedback to clients.

Guide clients through the analysis and interpretation of College of Nurses of Ontario (CNO) letters, including Letters of Assessment, the registration process (RNCCAP, CAS, academic pathways, language fluency, and nursing currency).

Assess clients’ financial, psychosocial, and settlement needs and provide information and referrals for funding applications or community agencies, as appropriate.

Assess clients’ language needs and make referrals to CARE Centre courses and/or language deliveries in the community, as appropriate.

Develop an individualized CARE Centre action plan based on the results of the assessment and consideration of all appropriate resources for the client.

Monitor the progression of the plan on a regular and ongoing basis and revise the plan as needed.

Provide post-exam analysis and create appropriate learning plans for clients.

Assist clients with job search tasks (resume writing, interviewing skills, labour market research) directly or through referral to appropriate community-based agencies.

Provide character references for clients.

Intercede and/or advocate for clients with Ontario Works (OW), CNO, settlement service agencies, academic institutions, and other community partners, as needed.

Assist the client in developing cross-cultural competencies and the ability to manage acculturation issues, particularly in the workplace.

Implementation of Project Activities

Support the Project Coordinator in implementation of project activities according to project timelines and deliverables.

Coordinate with the Project Coordinator the project activities with participating Long-Term Care employers and community partners.

Support recruitment and onboarding of IEN participants.

Coordinate orientation sessions, educational workshops, mentoring activities and employer engagement initiatives.

Maintain project documentation, tracking systems and participant records.

Monitor participant progress and project milestones.

Assist with preparing progress reports for funders and project partners.

Participate in project evaluation activities, including surveys, interviews and outcome measurement.

Identify implementation challenges and recommend solutions.

Outreach and Employment Engagement

Implement CARE Centre’s community-based outreach strategy through developing relationships with, and promoting awareness of CARE Centre‑Nurse Reach Ontario Long-term Project services to, a range of organizations, and support the Executive Director and Project Coordinator in building alliances or regional partnerships in Long-term Care in Ontario, as appropriate.

Outreach to the network of LTC organizations in Ontario to advocate for IENs’ position or identify new or innovative services for IENs, as appropriate.

Support development of educational resources and project materials.

Participate in presentations and dissemination activities.

Coordinate with the Employment Engagement Lead:

Build and maintain relationships with participating Long-Term Care organizations.

Collaborate with employers to support recruitment, onboarding and workplace integration.

Promote CARE Centre's Workplace Integration Program and related resources.

Facilitate communication between employers and IEN participants throughout the project.

Other Duties as assigned QUALIFICATIONS

Degree in Nursing, Education, Social Services with experience in health care, settlement, or education.

Highly developed case management skills.

Keen understanding of the barriers faced by IENs in Ontario and a demonstrated capacity to develop and implement programs to effectively respond to these barriers with focus on LTC.

Experience in, and understanding of, the Ontario health care system and Long-term care sector‑legislation and regulations.

Cross-cultural competency.

Demonstrated facilitation skills.

Strong counseling skills.

Excellent communication skills in English – written, verbal, and presentation skills.

Program planning skills and experience.

Ability to work flexibly within a dynamic and highly evolving environment.

Experience collecting and reporting project performance indicators.

PHYSICAL DEMANDS

Light, infrequent medium physical demands for lifting, pushing, pulling.

Manual dexterity required to use computer and office equipment.

WORKING CONDITIONS

Flexibility within program operating hours.

Possible travel to other locations in Ontario, travel to conferences and meetings.

SALARY RANGE $72,000 to $75,000
