Part Time Care Coordinator - Health Services

Enhance lives as a Part Time Care Coordinator with the Intake and Access Team, focusing on patient care and community service access. This role requires a skilled registered nurse, physiotherapist, occupational therapist, social worker, dietitian, or speech-language pathologist ready to assess and determine patient care needs. Key Responsibilities:
• Assess patient care needs and eligibility for services
• Develop and coordinate individual care plans with stakeholders
• Link patients with appropriate service providers
• Establish supportive relationships with patients and families
• Monitor care plan delivery while respecting patient preferences Requirements:
• Relevant regulatory body membership in Ontario
• 1+ years of recent community health experience
• Knowledge of health care delivery systems
• Exceptional interpersonal and organizational skills
• Valid driver’s license with access to a vehicle Leverage your expertise in care coordination, problem-solving, and patient engagement to create impactful connections in community health.
