Home Care Licensed Practical Nurse

Provide essential nursing services as a Licensed Practical Nurse in a community setting. This role demands accountability for case management and requires travel to client homes up to 100 km/day.

As a Licensed Practical Nurse in Home Care, you will join an interprofessional health care team to deliver nursing care across the lifespan. Your responsibilities include assessing client needs, coordinating care plans, and performing interventions based on identified service requirements. This role is vital for clients, particularly those with complex needs.

Key Responsibilities: • Manage assigned caseload and assess client needs • Coordinate transitions and create individualized care plans • Perform nursing interventions as necessary • Document care and communicate with the team using mobile technology • Travel to client locations, providing care across rural areas

Requirements: • Completion of an accredited practical nurse program • Active or eligible registration as a Licensed Practical Nurse • Minimum two years of nursing experience in Acute Care • Current Basic Cardiac Life Support certification • Ability for physical demands including lifting and mobility
