Overview

Registered nurses provide home‑based care with a per‑visit model, adaptable scheduling, paid training, and travel to client homes. This role requires delivering evidence‑based nursing care and coordinating with a care team. Responsibilities

Complete comprehensive client assessments, develop care plans, implement and evaluate care based on best evidence and the client’s needs. When designated as a primary provider for a client caseload, act as the point of care lead and collaborate with other care team members and support functions. Support the client through care transitions by collaborating and connecting the client with community support services in their local community. Qualifications

Current registration with the College of Nurses of Ontario CNA specialty certification is an asset Enthusiasm for community care and a commitment to evidence‑informed practice and excellence in care Current CPR certification is required Valid driver’s license and vehicle (travel required) Updated immunization records, transparent criminal background check, and vulnerable sector check are required upon hire Willingness to attain required Saint Elizabeth certifications Availability to work days and weekends Physically demanding position requiring dexterity, lifting up to 55 pounds, standing, bending, reaching, and frequent movement
