Registered Nurse - Continuing Care (Edmonton)

About the Role

Assigned to the Home Living team in Edmonton, the Registered Nurse (RN) will provide essential nursing services in clients' homes across all ages and life stages. The RN works as a self‑directed member of the Continuing Care team, collaborating with health‑care professionals and families to assess needs, deliver therapeutic nursing services, coordinate resources, and establish and evaluate care goals. The RN also provides case management, direct client care, and clinical expertise, guiding team members, developing care plans, and mentoring student clinical experiences. Key Responsibilities

Conduct thorough assessments of clients’ needs and develop individualized care plans Deliver therapeutic nursing services consistent with professional practice standards Coordinate internal and external resources to meet client goals Provide case management and direct client care that is evidence‑based and client‑centered Advise the multidisciplinary team on clinical matters and influence practice standards Participate in staff development and mentor student clinical experiences Advocate for patients and families, promoting health and safety through education and counseling Required Qualifications

Completion of an accredited nursing education program Active registration and practice permit with the College of Registered Nurses of Alberta (CRNA) Current Basic Cardiac Life Support – Health Care Provider (BCLS‑HCP) certification For candidates not practising RN within the past five years: completion of the required refresher courses (CRNA Medication Management Chapters 1‑4 and NextGenU Health Assessment Resources Certification) At least two years of recent nursing experience within the last five years, or completion of a refresher program within two years Self‑directed, independent work style using sound professional judgement Strong organizational, communication, and assessment skills appropriate for a fast‑paced, home‑care environment Proficiency in computer skills, including Microsoft Office, Visio, Outlook and Internet; adequate typing speed Physical capability to perform nursing tasks, including client transfer and lifting medical equipment (25–30lb) Capacity to work independently in a variety of home settings and weather conditions Preferred Qualifications

Baccalaureate degree in nursing Experience in palliative care, geriatric care, or previous Continuing Care Familiarity with Connect Care and the RAI Home Care assessment tool At least two years of current case‑management experience in Continuing Care
