Position Summary

The Team Lead, Adult Housing Supports (AHS), provides day-to-day leadership, supervision, and support to Adult Housing Supports staff while ensuring the effective delivery of participant-centered services. Working collaboratively with the Program Manager and fellow Team Leads, the role supports program planning, service coordination, case planning, and capacity building to achieve program outcomes and maintain continuity of care. Using a recovery-oriented approach, the Team Lead promotes high-quality service delivery, supports staff development, and serves as a key liaison between the program and community partners.

Schedule

  • Monday to Friday; 8:00 AM – 4:30 PM
  • Some flexibility in scheduling is required based on program needs

Key Responsibilities

  • Provide day-to-day leadership, supervision, coaching, and performance management for assigned Adult Housing Supports staff.
  • Support staff in case management, goal planning, housing stability, and recovery-oriented service delivery.
  • Coordinate referrals, caseload distribution, and service planning to ensure continuity of care and effective program operations.
  • Conduct file reviews, monitor service quality, and ensure compliance with program standards, funding requirements, and organizational policies.
  • Build and maintain collaborative relationships with landlords, community agencies, service providers, and internal stakeholders.
  • Facilitate case conferences, service coordination meetings, and collaborative planning to support positive participant outcomes.
  • Monitor program performance, analyze data and outcomes, and contribute to program evaluation and continuous improvement initiatives.
  • Recruit, train, mentor, and support staff development through ongoing supervision, education, and capacity-building opportunities.
  • Provide guidance and support related to critical incidents, risk management, safety planning, and complex participant situations.
  • Ensure accurate documentation, reporting, financial accountability, and administrative processes in support of program objectives.

Qualifications

Education

  • Diploma or Degree in Social Work, Human Services, or related field.

Certification

  • Suicide Intervention
  • Standard First Aid and CPR
  • Valid Class 5 Driver’s License and vehicle
  • Proof of vehicle insurance (min. $2 million liability), registration and 3-year drivers abstract
  • De-escalation

Experience

  • 4+ years of experience supporting marginalized, high-risk, and vulnerable populations.
  • 2+ years of progressive leadership experience, including staff supervision, coaching, and/or change management.
  • Strong knowledge of Recovery-Oriented Systems of Care (ROSC), homelessness, harm reduction, mental health, addictions, trauma, sexual exploitation, and culturally responsive practices.
  • In-depth understanding of community resources and supports related to health, mental health, addictions, legal services, income supports, housing, and family services.
  • Proficiency with Microsoft Office and database systems, with the ability to maintain accurate records and report on outcome-based programs.
