Join Vancouver Coastal Health’s Home Health team as a Dietitian in Vancouver, BC. Engage in client-centered nutrition care and collaborate with interdisciplinary teams for optimal health outcomes. As a Home Health Dietitian, you will work independently and within a team to create and implement nutrition care plans tailored to clients' needs. Your role involves contributing to community interventions against malnutrition and providing evidence-based nutritional guidance. Engage with clients across various demographics, including the frail elderly and younger individuals facing health challenges. Key Responsibilities:
• Develop and evaluate individualized nutrition care plans
• Implement community nutrition education and support programs
• Consult with staff and community agencies on nutrition issues
• Advocate for clients’ nutritional needs in care settings
• Assess and respond to client feedback effectively Requirements:
• Registration as a Dietitian with CHCPBC
• Two years of related experience or equivalent
• Valid BC Driver's License for local travel
• Competency in enteral nutrition practices
• Knowledge of dietetic standards and evidence-based practices Leverage your dietetic skills to improve community health with Vancouver Coastal Health.
