Responsibilities

• Provide disease and therapy information to patients and healthcare providers based on approved materials
• Assess patient eligibility and coordinate enrollment in patient support services
• Act as liaison between patients, families, and healthcare providers for medication access issues
• Document patient interactions and conduct ongoing file reviews within electronic records
• Monitor therapy adherence, track adverse events, and collaborate with reimbursement specialists on funding

Requirements

• Current nursing license and registration in Quebec
• Fluency in English and French
• Minimum 3-5 years case management or patient support experience
• Knowledge of pharmaceutical therapies and patient support programs
• Proficiency with electronic health records and data management
• Strong communication and interpersonal skills
