Infirmière Gestionnaire de Cas, Bilingue - Nurse Case Manager - Bilingual

Cencora seeks a bilingual licensed nurse to manage patient cases and provide support services for pharmaceutical programs in Quebec. The role involves coordinating care between patients, providers, and reimbursement specialists while ensuring adherence and expedited therapy access.
Responsibilities
- Provide patient and provider support regarding medication access, disease information, and therapy-specific guidance
- Conduct patient assessments and act as liaison between patients, families, and healthcare providers
- Document patient interactions and maintain electronic records with ongoing file reviews and updates
- Follow case-management protocols including adverse event reporting and therapy monitoring
- Coordinate with healthcare providers, pharmacies, and clinics for treatment logistics and issue resolution
Requirements
- Licensed Nurse (RN or RPN/LPN) with active provincial registration required
- Diploma in healthcare or related field; bachelor's degree preferred
- Bilingual proficiency in English and French
- Problem-solving and independent work capabilities
- Strong analytical and team collaboration skills
- Ability to deliver high-quality work in dynamic environments
