Expected Start Date: July 21, 2026 Union: HSAS Facility: Saskatoon Home Care Office City/Town: Saskatoon Department: CPAS - Community C Type: Full-time temporary Expected Up to Date: June 18, 2027 FTE: 1 Shift Information: Days, Evenings, Weekends Number of Hours per Rotation: 14 shifts of 8 hours per 3 weeks Relief: No Float: No Hours of Work: Standard Hours Salary or Pay Band: Pay Band Assessor Coordinator - Degree $36.721 to $44.902 Travel Required: No Job : The overall function of the assessor/coordinator (client care coordinator) position is to provide case management services which includes assessment, planning, monitoring and evaluation. The client/patient access services is primarily responsible for a single entry access and ongoing coordinator for a variety of community based services within SHA (Saskatoon). "Preference for this position will be given to HSAS members in accordance with the terms and conditions of the SAHO/HSAS Collective Agreement" Human Resources Exemption: No Education- Bachelor of Social Work (BSW)
