Expected Start Date: July 13, 2026 Union: HSAS Facility: Royal University Hospital City/Town: Saskatoon Department: Continuing Care Program Integration Type: Full-time temporary Expected Up to Date: July 13, 2027 FTE: 1 Shift Information: Days Number of Hours per Rotation: 14 shifts of 8 hours per 3 weeks Relief: Yes Float: No Hours of Work: Standard Hours Salary or Pay Band: Pay Band Assessor Coordinator - Degree $36.721 to $44.902 Travel Required: No Job : The overall function of the assessor/coordinator (client care coordinator) position is to assess and coordinate single entry access to personal care homes and long term care. "Preference for this position will be given to HSAS members in accordance with the terms and conditions of the SAHO/HSAS Collective Agreement" Human Resources Exemption: No Education- Bachelor of Social Work (BSW)
- Bachelor of Science in Nursing (BScN)
Other Education and Training
- Bachelor of Social Work is acceptable
- Bachelor of Nursing is acceptable
Knowledge and Abilities
- Knowledge in the SHA, community resources and applicable programming

- Previous Client Assessment Experience is an Asset
- Valid driver's license, where required
- Travel required
- Registered/eligible with the College of Registered Nurses of Saskatchewan (CRNS)
- Registered/eligible with the Saskatchewan Association of Social Workers (SASW)
