Clinical Pharmacist – Markham Stouffville Hospital, Uxbridge Hospital & Reactivation Care Centre

As a clinical pharmacist at Oak Valley Health, you will provide clinical pharmacy services to patients at Markham Stouffville Hospital, Uxbridge Hospital, and the Reactivation Care Centre. You will work with the inter‑professional team to facilitate medication therapy management, patient education, and support for nursing and medical staff.

Responsibilities

• Provide drug distribution and clinical pharmacy services across multiple clinical areas including Surgery, Medicine, ICU/CCU, Childbirth, Psychiatry, Rehabilitation, Palliative, Complex Care, Oncology, Emergency, Day Surgery, and others.
• Participate in patient education and medication counseling.
• Support teaching students and provide drug information to nursing and medical staff.
• Collaborate with pharmacists and pharmacy technicians as an integral member of the pharmacy team.

Qualifications

• Graduate of an accredited university pharmacy program.
• Licensed under Part A registration status with the Ontario College of Pharmacists; hospital pharmacy residency preferred.
• Previous experience in a hospital setting with unit‑dose and IV admixture programs.
• Demonstrated experience identifying, resolving, and preventing drug‑related problems to optimize medication therapy and safety.
• Demonstrated computer efficiency.
• Excellent communication and interpersonal skills and ability to work collaboratively with an interdisciplinary team.
• Evidence of professional development, continuing education, and leadership experience preferred.
• Good attendance and performance record.
• Commitment to a healthy and safe workplace, complying with hospital policies, health and safety legislation, and mandatory education requirements.

Work Schedule

8‑hour shifts days, evenings, and weekends (Monday–Sunday). On‑calls are required and schedules may change due to operational needs.

Compensation

OPCOM09 Range: $51.29 – $67.47 per hour.
