Palliative Care Nurse Practitioner Ontario Health

• Deliver primary health care services to at-risk patients
• Collaborate effectively with physicians and care coordinators
• Facilitate patient education on health management
• Conduct focused assessments to guide care initiatives
• Promote effective coordination of health services Requirements:
• Valid standing with the College of Nurses of Ontario
• RN EC designation required
• Knowledge of health care protocols and community roles
• Palliative or end of life care experience preferred
