Patient Care Assistant, Medical & Renal Care

As a Patient Care Assistant, Medical & Renal Care, you will combine practical assistance with daily living activities and the compassionate psychosocial support that helps patients feel dignified and supported throughout their treatment. Your work ensures that fundamental needs, from hygiene to emotional stability, are met with consistency and respect. By integrating safety observation with timely communication, you provide the critical updates that allow the interdisciplinary team to maintain a safe and responsive care plan.

Your ability to de-escalate anxiety through calm dialogue ensures that patient’s feel supported rather than isolated during their hospital stay.

You recognize that patients living with complex medical conditions need time and space to adapt, and that progress is often gradual or non-linear. By meeting patients where they are, you provide consistent, steady support without pressure. This approach helps patients feel respected as they navigate new diagnoses, manage chronic conditions, or adjust to changes in their functioning. Through a calm and encouraging presence, you foster trust and support the resilience patients need to manage the complexities of their care.

In this role you will:

• Provide constant observation of assigned patients, ensuring their safety and well-being
• Deliver compassionate psychosocial support, fostering a reassuring environment that contributes to the emotional comfort of patients
• Accompany and assist patients during transfers to external medical appointments
• Execute patient care tasks under the collaborative direction and guidance of the nursing team to support comprehensive treatment plans
• Assist patients with active daily living activities, including bathing, toileting, and dressing
• Maintain accountability by ensuring accurate documentation of care and sustaining ongoing communication with the health care team regarding patient needs
• Recognize and report early warning signs of escalating high-risk behaviours and initiate de-escalation techniques as required
• Notify and escalate concerns to the most responsible regulated health professional as per established protocols if patient(s) require assistance, are engaged in potentially unsafe behaviours, or appear to pose a risk to themselves or others
• Support patient’s nutrition and hydration; maintain awareness of patient nutritional status and dietary restrictions
• Perform other duties consistent with the job classification as required

Job Requirements:
Mandatory

• Successful completion of a Personal Support Worker (PSW) program from an accredited College
• Currently registered and in good standing with the Health and Supportive Care Providers Oversight Authority (HSCPOA)
• CPR (Heartsaver or BCLS), First Aid Certificate

Preferred

• Previous experience in caring for the elderly and those with chronic complex diseases and medical conditions 

Skills and Knowledge

• Basic knowledge of established Infection Control and Occupational Health & Safety practices and knowledge of body mechanics, and safe lift and transfer practices
• Basic knowledge of sterility and aseptic technique
• Basic knowledge of how to meet patient needs and the ability to perform a range of patient care activities with the scope of the role
• Demonstrated proficiency in communication skills, both verbal and written
• Demonstrated satisfactory job performance and attendance history required
