Registered Practical Nurse, Medical Rehab Care

As a Registered Practical Nurse, Medical Rehabilitation, you’ll support patients in regaining their independence, working with a team to create individualized care plans that bridge the gap from hospital recovery to life in the community. Your practical nursing skills will be applied in diverse and challenging situations, requiring both critical thinking and creativity. By collaborating with an interdisciplinary team, you’ll ensure that each patient receives the tailored support they need for successful rehabilitation. About the Medical Rehabilitation (8 North) team
Hennick Bridgepoint Hospital’s Medical Rehabilitation Unit (8 North) is a 32-bed unit with one flex bed, caring for patients transitioning from acute care with a wide variety of diagnoses. We are a close-knit, supportive team that is always adapting to the changing needs of our patients. Together, we develop personalized care plans that focus on restoring function, mobility, and improving quality of life. We pride ourselves on maintaining open communication, with each team member contributing their expertise toward a common goal: helping patients regain independence. In this role you will:

• Practice independently and accountably within the RPN scope of practice, adhering to CNO standards and Sinai Health policies
• Identify changing patient needs to initiate timely consultation and collaboration with Registered Nurses and the wider interprofessional team
• Monitor and document the progress of clinical care plans and outcomes, adjusting strategies as needed to optimize treatment goals and patient safety
• Participate in the mentoring of learners, peers and other regulated and unregulated health professionals
• Provide health education to patients and their families using effective teaching strategies
• Assess spiritual, cultural and psychosocial needs of patients
• Provide comprehensive and accurate documentation of assessments, care provision, and evaluations
• Collaborate with the patient, significant others and multidisciplinary team in the provision of care
• Provide comprehensive and accurate documentation of assessments, care provision, evaluations, discharge and transition planning
• Perform other duties consistent with the job classification as required

 

Job Requirements:
Mandatory

• Graduate of a CNO-approved Practical Nursing (PN) program 
• Currently registered as an RPN and in good standing with the College of Nurses of Ontario (CNO)
• Current Basic Cardiac Life Support (BCLS) certification

Preferred

• Experience with electronic health record (EHR) and clinical documentation
• Previous medical and/or surgical clinical experience

• Effective time-management and organizational skills
• Excellent problem solving and critical thinking skills
• Strong collaboration skills to build professional relationships and support integration with health care team

• Demonstrated satisfactory job performance and attendance history required
