Job Overview  

Reporting to the Associate Director of Care, the Registered Nurse serves Kensington Health’s LTC home (i.e. Kensington Gardens) to support and advance quality of care, safety and the experience of the people we serve.  

The role is pivotal to ensuring the day-to-day management of the Resident Home Areas (RHAs) and the management of the health care and services required to meet the needs of the people we serve.  

This role will be guided by the legislation and regulations governing LTC in Ontario, the policies and procedures and best practices in accordance with the Standards of Practice of the College of Nurses of Ontario. 

Responsibilities

Clinical Operations:  

  • Coordinates the RPN and PCA team on the assigned RHA to provide effective care for all residents on the Resident Home Area consistent with each resident’s care plan. 

  • Assists RPN on completing clinical assessments for unstable and or with a change in health condition, palliative, end of life. 

  • Assists RPN with new admission /re-admission from hospital. 

  • Assists RPN with residents’ emergencies and /or other emergencies as they arise. 

  • Follows up on critical results and ensures physician /NP are notified. 

  • Support RPN to identify and manage clinical risks (responsive behaviors, falls). 

  • Empowers the Personal Care staff with the responsibility to plan effective care for each resident with resident and family input. 

  • Follows to ensure resident and family is aware of resident care, changes in health condition, changes in medications, incidents. 

  • Serves as resource person for other Personal Care staff in identifying and solving resident care problems. 

  • Evaluates the care provided and participates with other Personal Care personnel in evaluation of resident care. 

  • Communicates with and supports families in their involvement and participation. 

  • Assist in planning assignments for RPN and PCAs as directed. 

  • Provides clinical assessments on adjacent RHA for medically unstable residents and any assessment of residents at the request of an RPN in the Home. 

  • Promotes an environment in which the health care team works co-operatively and supportively. 

  • Uses scientific knowledge in a systematic approach to identifying problems and in problem solving. 

  • Communicates and documents professional practice. 

  • Ensures that the appropriate Senior Management is apprised of any problems in a timely fashion as delegated by the Director of Resident Care. 

 

Quality, Safety and Experience:  

  • Demonstrates a commitment to the philosophy and objectives of the Personal Care services within the Kensington Health Centre. 

  • Promotes the Resident’s Bill of Rights. 

  • Respects and uses sensitivity in spiritual, cultural and ethnic aspects of a resident’s life. 

  • Promotes person-centered practices and approaches in all resident encounters and decisions. 

  • Provides input to the development and revision of Personal Care policies and procedures and communicates changes to staff. 

  • Promotes an environment in which the health care team works co-operatively and supportively. 

  • Participates in the Home programs (e.g.) Occupational Health and Safety Program, Quality Improvement, Fire Safety, WHMIS. 

  • Gives input into any revisions to the job descriptions and functions of members of the Personal Care Service. 

  • Seeks out opportunities to maintain competency and to continuously learn, taking an active role in continuous quality improvement. 

  • Encourages and assists Personal Care personnel to identify learning needs. 

  • Is flexible in the event of an unforeseen staffing shortage and will assume responsibility for an additional RHA as assigned.  

  • As a condition of employment completes all mandatory education topics as required by the Long-Term Care Homes Act, either by attending in-house education sessions or via the home’s online learning website. 

  • Responsible for working safely in compliance with accepted safe work practices, procedures and legislated health and safety standards. 

  • Responsible to use and wear the equipment, protective devices and personal protective equipment as required; report hazards, accidents, and the absence of or defect in any equipment or protective device; attend all required health and safety training; and follow all established safe work procedures.

Human Resources:  

  • Conducts performance appraisals of Personal care Attendants in the assigned Home Areas. 

  • Promotes a co-operative working environment. 

  • Facilitates team building and promotes change and innovative ideas. 

  • Gives input to ADOC and Health and Safety team to support team members who require modified duties and accommodations in workplace. 

 

Financial and Material Management:  

  • Provides input into the need and use of equipment and supplies required for residents. 

  • Ensures health-related equipment and supplies are well-maintained and sufficient in number.  Provides input to the ADOCS as to needs identified. 

  • Provides input to nursing management team when exploring new purchases of supplies and/or equipment to augment current practices. 

 

Qualifications

Required Skills and Abilities:  

  • Multilingual facility an asset. 

  • Facilitating in both official languages would be a definite asset. 

 

Required Knowledge and Experience:  

  • Minimum of one year experience in geriatric, rehabilitation or long-term care as charge nurse is preferred. 

  • Demonstrates an understanding of current trends and concepts relative to professional Nursing. 

 

Required Professional Designation/Certificate:  

  • Current certificate of competence from the College of Nurses of Ontario. 

  • Certification in Gerontology and Current membership with GNA and RNAO is desirable.
