As a Clinical Pharmacist, you will be integral to patient care at the White Rock Primary Care Clinic. You will carry out assessments, recommend therapies, and implement care plans while collaborating closely with prescribers. Your skills in pharmacotherapy, communication, and adaptability will ensure effective medication management. Key Responsibilities:

• Identify and resolve drug therapy problems with patients
• Provide comprehensive medication management and monitoring
• Develop patient-specific therapeutic plans in collaboration
• Maintain documentation according to organizational standards
• Participate in quality improvement activities and patient safety programs

Requirements:

• Current registration as a Pharmacist in British Columbia
• Two years of relevant patient care experience required
• Valid Class 5 driver's license and personal vehicle access
• Strong communication and problem-solving skills
