As a Clinical Pharmacist, you will provide patient-centred medication management while collaborating with healthcare professionals. Your role includes developing drug therapy guidelines, conducting medication evaluations, and ensuring precise dispensing of medications. Key Responsibilities:
- Discuss therapeutic plans with healthcare team members
- Customize medication therapy for individual patients
- Educate patients on their medications effectively
- Mentor pharmacy students and contribute to case studies
- Participate in quality improvement and safety initiatives
- Licensed as a Pharmacist in British Columbia
- Background in Hospital Pharmacy Residency or equivalent
- Comprehensive knowledge of pharmacy laws and practices
- Skilled in written and verbal communication
- Capability to track and manage drug inventory accurately
