Social Work - Young Adult Mental Health Program

Reporting to the Manager of Ambulatory Paediatric Mental Health and Manager of Health Disciplines, this position will work providing psychotherapy to the Young Adults Mental Health Program in an interdisciplinary team approach.

The Social Worker will work with outpatients and day patients to ensure continuity of care, as well as liaise with family doctors, schools, community agencies, various areas of the hospital, and colleagues within the Young Adult Mental Health Program. This role will require building strong therapeutic relationships with patients and families to support assessing risk, safety planning, and building commitment to treatment.

Essential to this role is understanding practice is based in theory, ensuring practice is evidence based and meets all relevant standards and guidelines as outlined by the College of Social Workers.

This is a formative time to come into working in Adolescent Mental health, as Children's Hospital is refreshing how we work to serve our patients and families in the age group of 17.5 years to 25 years. We invite the successful candidate to be actively present with patients, families, staff and partners to make meaningful change to best serve our community where they are at. This position will have the opportunity and expectation to be working offsite with and at the locations of our care partners in the community.

Qualifications

• Master's Degree in Social Work from an accredited university
• Current registration with the Ontario College of Social Workers and Social Service Workers
• Demonstrated knowledge with the Mental Health Act; Substitute Decision Makers Act and Health Care Consent Act preferred
• Skilled in psychosocial assessment and counseling (including individual, family, group
• Minimum of two years recent experience within paediatric mental health care required
• Completion of DBT training and practice working with clients individually
• DBR level 1 certification preferred
• Completion of ITCT-A training and experience of implementation of the therapy required
• Demonstrated ability to identify and Implement strategies with patient/family by establishing a therapeutic relationship in concert with proven treatment modalities (for example Crisis Intervention, Cognitive-Behavioural Therapy, Critical Incident Stress Management, Solution-focused Therapy, etc.)
• Demonstrated excellent clinical assessment and treatment skills
• Demonstrated excellent communication skills (both oral and written)
• Demonstrated skill in crisis assessment and intervention
• Demonstrated knowledge of community services and resources and experience with community networking
• Demonstrated strength in interpersonal relations, problem-solving, crisis intervention, group dynamics, community development and other innovative approaches
• Demonstrated ability to function independently and interdependently with a diversity of health care team members
• Demonstrated skill in providing education to health care team and community resources
• Demonstrated ability to measure and evaluate practice change outcomes
• Demonstrated practice and commitment to the principles of patient and family centered care
• Demonstrated practice and commitment to patient and staff safety at LHSC
• Demonstrated practice and commitment to LHSC's vision, purpose and values
• Demonstrated ability to attend work on a regular basis
