This position is responsible to provide intake assessment and service for all program eligibility/ ineligibility within Social Programs. As part of the Social Programs integrated client centered service delivery model, this position provides information, problem solving support and advocacy through phone advice & face to face to individuals and families experiencing difficulties related to financial and personal functioning. The programs to which the Social Program Intake Screener would be responsible to deliver are: Social Supports; Child Care Subsidy; and AccessAbility Supports. The Social Program Intake Screener will work collaboratively with colleagues, community and applicants and facilitate referrals across all programs and external agencies. The Social Program Intake Screener provides specialization in the assessment and delivery of services with knowledge and expertise on the support needs of people with multiple barriers and working within a multi-disciplinary division.
Duties will include but are not limited to:
- Conducting intake screening assessments for services of Social Programs applicants via phone or in-person, responding to inquiries regarding program guidelines, and assessing needs and risk factors during interviews.
- Identifying situations requiring immediate intervention or referral, including emergency financial needs that may require Social Assistance.
- Managing application documentation, including explaining requirements (such as Health Reports and Disability Support Information), providing support to applicants in obtaining necessary forms, and overseeing accurate record maintenance for eligibility reviews.
- Determining eligibility in accordance with Program Acts/Regulations, policies, and procedures.
- Analyzing presented information to confirm accuracy and evaluate the need for appropriate follow-up.
- Coordinating the eligibility and clinical assessment process, liaising with the eligibility committee and Clinical Assessors, booking clinical assessments, and scheduling intake appointments for the Support Coordinator.
- Developing clear and concise written correspondence regarding complex issues related to program eligibility.
- Other duties as required.
Minimum Qualifications
- The incumbent must have a University Degree in a discipline related to the duties of the position preferably in Social Sciences.
- Experience working with individuals and their families in the area of social services.
- Experience working in social programs is preferred.
- Demonstrated equivalencies will be considered.
- Strong knowledge and understanding of community resources, including but not limited to Child and Family Services, Mental Health Services, Addiction Services, School systems, Police, Housing, medical services, counselling services, Family Violence workers, shelters, youth justice etc.
- Knowledge of the legislative requirements, regulations, policies and procedures and the mission, beliefs and vision of the Social Programs Division.
- Requires skills in risk assessment, counselling, critical thinking, group work and crisis intervention.
- Ability to work independently with minimal supervision.
- This position requires excellent oral and written communication skills.
- Excellent interpersonal, time management, problem solving and organizational skills.
- Requires computer literacy with experience using a variety of software programs ie. Outlook, ISM, word perfect, Microsoft word.
- Valid PEI Driver’s license.
- Provide a satisfactory Credit Check. You will be asked to complete a consent form authorizing HR to request a credit check through our third-party provider.
- A good previous work and attendance record and an acceptable criminal record check and vulnerable sector record check.
- This is a bilingual position. Advanced oral proficiency in English and French is required.
