- Conduct essential patient assessments in primary care
- Perform personalized hormone replacement therapy
- Collaborate with healthcare teams for treatment plans
- Educate patients and families on disease management
- Accurately document care in EMRs for continuity
- Master of Science in Nursing and APRN certification
- Valid Nurse Practitioner license for California
- 2+ years of primary care experience required
- Minimum 2 years’ practical experience in Functional Medicine
- Excellent clinical judgment and communication skills
