JOB SUMMARY: The Manager of Home and Community Care reports to and works under the direction and supervision of the Senior Manager, Primary and Clinical Care.

To assist the Department of Well-Being in the performance of statutory, operational and advisory duties related to the Home and Community Care Program and to provide leadership and operational oversight for assigned Home and Community Care programs, services and staff.

The individual will assist in planning, implementing, coordinating, developing and maintaining Home and Community Care programs and services within the policies and procedures established by the Six Nations of the Grand River Elected Council. The Manager is responsible for supporting effective day-to-day service delivery, staff supervision, program quality, client safety and the ongoing development and evaluation of assigned services to meet the needs of the Six Nations community.

BASIC QUALIFICATIONS:

• Bachelor’s Degree and three years of related experience in a health setting OR College Diploma and three years of related experience including supervision of support staff in a health setting


• Demonstrated leadership and staff supervision skills, including the ability to coach, support and manage staff within a multidisciplinary environment.


• Knowledge of palliative care


• Demonstrated experience working in an inter-disciplinary team


• High level of computer skills – Microsoft Office – MS Word and Excel

POSITION TITLE:  Manager of Home and Community Care

REPORTING RELATIONSHIP:

Reports to and works under the direction and supervision of the Senior Manager, Primary and Clinical Care.

PURPOSE & SCOPE OF THE POSITION:

To assist the Department of Well-Being in the performance of statutory, operational and advisory duties related to the Home and Community Care Program and to provide leadership and operational oversight for assigned Home and Community Care programs, services and staff.

The individual will assist in planning, implementing, coordinating, developing and maintaining Home and Community Care programs and services within the policies and procedures established by the Six Nations of the Grand River Elected Council. The Manager is responsible for supporting effective day-to-day service delivery, staff supervision, program quality, client safety and the ongoing development and evaluation of assigned services to meet the needs of the Six Nations community.

KEY DUTIES & RESPONSIBILITIES:

Technical Functions:

• Approve tenders for the purchase of equipment, materials and services; examine tender proposals to ensure that they meet the specified needs and recommended actions.


• Program planning, development and evaluation.


• Ensures programs are developed to meet the needs of the community through review, modification and evaluation.


• Prepares program reports for the department, Central Administration, Six Nations of the Grand River Elected Council and funding agencies as required.


• Support the identification and acquisition of funds for capital and operational needs of the program


• Responsible for being familiar with funding agreements, eligible expenses and reporting requirements.


• Monitors program budgets to ensure balancing of statements and to ensure programs are operating within budget.


• Identify and support provision of necessary training for team members.


• Monitors day to day operations and supports team members as required.


• Coordinate the delivery of services provided by providing supervision/direction/leadership and advice to the staff as required to ensure efficient and coordinated service delivery to clients by all service components. 


• Supervise staff and evaluate employee performance consistent with the policies and administrative processes of the Home and Community Care program.


• Provides leadership and oversight for client service delivery, including supporting staff in addressing complex client situations, service concerns and barriers to care.


• Promotes effective interdisciplinary collaboration and communication among Home and Community Care staff and other internal and external service providers involved in client care.


• Supports appropriate documentation, information management and use of applicable electronic health record systems in accordance with organizational policies, professional standards and privacy requirements.


• Identifies operational risks, client safety concerns and service delivery issues and takes appropriate action or escalates concerns to the Senior Manager as required.


• Supports continuous quality improvement activities, including the review of program data, incidents, service trends, client feedback and other information relevant to program performance.

Communications Functions:

• Establish and maintain communication with agencies external to Six Nations of the Grand River for the purpose of awareness and potential program expansion.


• Foster and maintain positive relations with Six Nations community services.


• Provide leadership in the identification, adoption and promotion of programs and services to enhance the health and well-being of the people of Six Nations community.


• Promotes open, respectful and effective communication within the Home and Community Care team and supports a collaborative, culturally respectful and professional work environment.


• Facilitates regular communication with staff regarding program operations, client service needs, organizational priorities, policies, procedures and changes affecting service delivery.


• Works collaboratively with other Department of Well-Being programs and organizational departments to support coordinated services and address operational or client service issues.


• Responds to client, family and community concerns within the scope of the position and supports appropriate follow-up and resolution.


• Support community awareness of services offered.


• Ensure community access to reliable educational materials and supports related to all program areas.


• Participate in committees and working groups as required.


• Work collaboratively to identify and resolve community service issues.


• Advocate for Six Nations community needs at every level of inter-agency communication.

Administrative Functions :

• Support financial processes within given authority and maintain documentation as required.


• Monitor the spending of budgets for Home and Community Care programs within administrative and financial guidelines.


• Provides day-to-day supervision of direct-report staff, including supporting attendance management, workload management, scheduling and operational coverage as applicable.


• Addresses employee performance, conduct and workplace concerns in consultation with the Senior Manager and Human Resources, as appropriate.


• Supports recruitment, selection, orientation, onboarding and retention of Home and Community Care staff.


• Ensures staff are informed of and supported in meeting applicable organizational policies, program procedures, documentation requirements and standards of practice.


• Coordinate the orientation of all new employees and student placements.


• Maintains appropriate program records and documentation and supports timely completion of required administrative, funding and operational reporting.


• Conduct annual performance evaluations for all direct report team members.


• Review and maintain program policies as required.


• Identify and manage projects as required to support the development of the program.


• Assist with planning, developing and implementing the program work plan consistent with community, department and council guiding documents, mission and principles.


• Adhere to organizational policies and procedures.

Other Functions :

• Assist in the research and preparation of statistical, financial, policy and other reports required by the Senior Leadership Team.


• Participates in quality improvement, accreditation, risk management, emergency preparedness and organizational initiatives relevant to Home and Community Care and the Department of Well-Being


• Monitor adherence to occupational health and safety procedures.


• Monitor and report issues and concern related to the operation of Six Nations Department of Well-Being facilities, as appropriate.


• Performs other job-related duties as may reasonably be required by the Home and Community Care Leadership Team.


• Provides support for the Six Nations Emergency Measures Plan by ensuring awareness of the Emergency Measures Plan and assistance as instructed by the Senior Leadership Team. 


• It is the responsibility of the employee to be aware of and adhere to all policies and procedures, including those that relate to client safety, staff safety and risk management.

WORKING CONDITIONS:

• Work requires extensive interaction with public and is subject to deadlines, interruptions, mental stress and unscheduled hours of work.


• This position requires inside work and requires some travel


• Ability to prioritize and work independently while managing multiple competing demands, operational pressures and time-sensitive responsibilities.
