Responsibilities

• Guide and mentor less experienced team members on case handling and escalation

Requirements

• Bachelor's degree in nursing, social work, health sciences, public health, or healthcare administration
• 4+ years of experience in care management, case management, or clinical support
• Bilingual proficiency (English and French implied by job title)
• Certification in case management or care coordination preferred
• Strong assessment and care planning capabilities
