Infirmière Gestionnaire de Cas, Bilingue/Nurse Case Manager

Responsibilities
- Provide disease and therapy information to patients and healthcare providers based on approved materials
- Assess patient eligibility and coordinate enrollment in patient support services
- Act as liaison between patients, families, and healthcare providers for medication access issues
- Document patient interactions and conduct ongoing file reviews within electronic records
- Monitor therapy adherence, track adverse events, and collaborate with reimbursement specialists on funding
Requirements
- Current nursing license and registration in Quebec
- Fluency in English and French
- Minimum 3-5 years case management or patient support experience
- Knowledge of pharmaceutical therapies and patient support programs
- Proficiency with electronic health records and data management
- Strong communication and interpersonal skills
