The Care Company is seeking an experienced, highly organized, and solutions-focused Registered Nurse to join our growing team as the Clinical Practice & Operations Lead .
This full-time, primarily office-based leadership role is ideal for an experienced nurse with a strong background in home and community care who enjoys supporting field staff, resolving clinical and operational challenges, strengthening care practices, and contributing to the continued growth of an organization.
Working closely with senior leadership, the Clinical Practice & Operations Lead will provide clinical guidance to field staff, oversee client-care concerns and risk alerts, support incident investigations, coordinate staff orientation and education, contribute to quality and compliance initiatives, and help ensure the smooth delivery of safe, reliable, and client-centred care.

Schedule and Work Location
- Full-time, permanent position.
- Monday to Friday, daytime hours.
- Primarily in-person at The Care Company’s Toronto office in East York.
- Approximately 90% office-based and 10% field-based.
- Local travel to client homes and other community locations throughout Toronto and the Greater Toronto Area is required.
Key Responsibilities
Clinical Leadership and Field Support
- Provide clinical oversight, guidance, and support to nursing staff and other members of the field-care team.
- Serve as a clinical resource for staff managing complex client situations, changes in condition, care concerns, and practice-related questions.
- Support clinical problem-solving, escalations, risk management, and operational decision-making.
- Provide clinical supervision, mentorship, coaching, and performance support to nursing and other field staff.
- Promote professional accountability, sound clinical judgment, and consistent adherence to evidence-informed care practices.
- Conduct field visits as required to support client care, staff supervision, competency development, or the investigation of clinical concerns.
Client Safety and Risk Management
- Monitor client risk alerts and clinical updates daily and coordinate timely follow-up to support safe and effective care.
- Assess reported concerns, identify potential risks, determine appropriate next steps, and ensure required actions are completed.
- Prepare, review, and investigate internal and external incident reports, including reports required by Ontario Health atHome, the Workplace Safety and Insurance Board, and other applicable organizations.
- Conduct fair, thorough, and timely investigations involving clinical incidents, client complaints, employee concerns, documentation issues, and service-delivery risks.
- Identify recurring concerns and recommend corrective actions, education, policy changes, or quality-improvement initiatives.
- Address client, family, and employee concerns promptly and proactively by identifying root causes, coordinating appropriate follow-up, and resolving concerns as close to the source as possible.
Client Care and Service Coordination
- Act as a liaison among clients, families, field staff, healthcare professionals, The Care Company, and Ontario Health atHome coordinators.
- Support new-client intake, onboarding, care planning, and service implementation.
- Collaborate with clients, families, field staff, and healthcare professionals to develop, implement, and maintain clear, accurate, and effective care plans.
- Work with clients and families to confirm care requirements, service schedules, clinical priorities, and the appropriate level of support.
- Support continuity of care by responding to service concerns, troubleshooting coverage challenges, and communicating relevant updates to the appropriate team members.
- Assist with client scheduling and service coordination when clinical or operational leadership support is required.
Documentation and Clinical Information Management
- Review and process new clinical documentation, referrals, orders, risk information, and client updates received through the HPG portal and other approved systems.
- Communicate relevant clinical changes and care requirements to field staff in a timely manner.
- Review and update care plans to reflect each client’s current condition, risks, preferences, and service needs.
- Ensure scheduled services remain aligned with each client’s approved care plan and current clinical requirements.
- Monitor documentation quality and follow up on incomplete, inconsistent, or outstanding clinical records.
Quality, Compliance, and Professional Practice
- Monitor staff adherence to company policies, clinical best practices, regulatory requirements, and applicable professional standards.
- Contribute to quality-improvement, client-safety, compliance, accreditation-readiness, and policy-development initiatives.
- Assist with clinical audits, documentation reviews, trend analysis, and corrective-action planning.
- Support the development, implementation, and evaluation of clinical policies, procedures, tools, and educational resources.
- Help ensure that The Care Company’s clinical practices remain safe, consistent, client-centred, and aligned with organizational and regulatory expectations.
Education, Orientation, and Staff Development
- Coordinate and support staff orientation, clinical onboarding, competency assessment, continuing education, and ongoing training.
- Identify learning needs arising from incidents, audits, client concerns, practice gaps, or changes in service requirements.
- Collaborate with internal leaders and educators to develop practical training and competency-development opportunities.
- Support staff through coaching, shadow shifts, skills laboratories, case reviews, and other learning activities as appropriate.
Operational Leadership
- Support the day-to-day coordination of clinical, administrative, and nursing operations.
- Respond to calls, troubleshoot service concerns, communicate changes, and help maintain reliable continuity of care.
- Work collaboratively with management, care coordination, scheduling, human resources, and field teams.
- Identify opportunities to improve operational efficiency, communication, documentation, and service delivery.
- Support additional clinical, administrative, and operational functions as required.
- Take on additional leadership responsibilities as the organization and position continue to evolve.

Requirements
- Current registration as a Registered Nurse in good standing with the College of Nurses of Ontario.
- Minimum of five years of progressive nursing experience.
- Minimum of three years of direct, field-based home and community care experience.
- Excellent written and verbal communication skills in English, including the ability to communicate confidently with clients, families, employees, healthcare partners, and senior leadership, and to prepare clear, accurate, and professional clinical documentation, incident reports, emails, and other correspondence.
- Strong understanding of the clinical, operational, and interpersonal complexities associated with providing care in clients’ homes.
- Broad clinical experience supporting diverse client populations, diagnoses, and levels of care.
- Experience investigating clinical incidents, complaints, client concerns, documentation issues, and other risk-related matters.
- Previous leadership, supervisory, care-coordination, quality-improvement, or administrative experience is preferred.
- Experience working with Ontario Health atHome programs, electronic medical records, HPG, or publicly funded home-care services is considered a strong asset.
- Relevant continuing education, specialized nursing courses, or certifications related to home and community care are considered assets.
- Excellent verbal and written communication, organization, problem-solving, and critical-thinking skills.
- Sound professional judgment and the ability to make confident and timely decisions.
- Ability to manage competing priorities and follow multiple concerns through to completion in a fast-paced environment.
- Strong computer skills and comfort learning and working with electronic clinical and operational systems.
- A valid Ontario driver’s licence and access to a reliable personal vehicle are required.
