Job Summary
We have an exciting opportunity as a Full Time Clinical Pharmacist within Lower Mainland Pharmacy Services for our Chilliwack Primary Care Clinic located in Chilliwack, BC where you can showcase your clinical pharmacy skills! The BC Ministry of Health is funding the Pharmacists in PCN program, which integrates clinical pharmacists into primary care teams across health authorities provinces wide. Clinical pharmacists will work closely with patients and prescribers to perform assessments, make recommendations, and implement care plans. Success requires self‑motivation, excellent communication, a strong clinical background, and adaptability to process improvements.
Clinical Pharmacist Services
- Direct patient care: Communicate with patients and the health care team to determine goals of therapy and collect information; identify drug related issues; formulate and implement a pharmacy care plan and monitor outcomes; provide medication information and care transition
- Respond to drug information requests and assist in the development of drug therapy guidelines using pharmacotherapy knowledge
- Follow, maintain and reinforce medication use policies and processes to ensure proper and safe dosing, storage, handling, dispensing, and/or administration of medications and compounded sterile products
- Participate in process improvements to ensure compliance with adverse drug reaction and medication error reporting programs, implement safety mechanisms, and continually assess patient safety during transitions of care
- Contribute to published literature on evidence‑based pharmacotherapy
- Provide preceptorship and supervision to pharmacy students and residents
Additionally, you will ensure prescriptions are validated, identify safety concerns, work with the health care team to resolve discrepancies or problems, document discussions or modifications to the medication therapy, and dispense medications.
Requirements to Join Our Team
- Current registration as a Pharmacist with the College of Pharmacists of British Columbia
- Valid Class 5 driver’s licence and access to a personal vehicle for business‑related purposes
- Two (2) years’ recent, relevant patient care experience, ideally in a team‑based primary care setting or equivalent combination of education, training and experience
Detailed Overview
Situated within identified primary care network locales, this position provides clinical pharmacy services to patients of the primary care network. As a member of an interprofessional team you will work directly with patients to identify and resolve actual and potential drug therapy problems through comprehensive medication management.
Responsibilities
- Identify and resolve actual and potential drug therapy problems in collaboration with the patient and other members of the primary health care team.
- Provide comprehensive medication management and pharmaceutical care by monitoring the medical condition(s) and ongoing drug therapy for effectiveness, adherence, adverse reactions, drug interactions and therapeutic, chemical and laboratory incompatibilities. Communicate pertinent findings, recommendations and plans to the patient and appropriate healthcare practitioners.
- Consult with patients and members of the primary care health team to develop patient‑specific therapeutic plans based on chart review, patient information and assessment of clinical responses.
- Maintain appropriate documentation in the health record in accordance with organizational standards and practices as required.
- Use appropriate resources to support professional practice and patient care including those available through provincial programs, such as academic detailing, optimal drug use initiatives, opioid stewardship and de‑prescribing.
- Establish linkages, as required, with health authority, community pharmacy services and other relevant community services to support continuity of care for patients.
- Provide drug information, consultative and other support services to medical, nursing, pharmacy and other primary care health team members by communicating with them on drug related issues, questions and patient specific therapeutic regimens.
- Conduct patient counselling and education regarding drug therapy.
- Provide in‑service education to the primary health care team where appropriate.
- Provide instruction and supervision to students such as University Pharmacy Undergraduates and Pharmacy Residents.
- Participate in various committees as appointed, to provide guidance and advice on drug therapy.
- Participate in quality improvement activities through reviewing current practice, identifying actual or potential problems and recommending and evaluating modifications.
- Maintain workload records and metrics as required.
- Participate in a variety of programs such as Patient Safety and Drug Reaction Reporting by collecting and compiling relevant data as applicable.
- Share the responsibility for orientation of new personnel and act as a role model.
- Maintain a working knowledge of PharmaCare policies and procedures.
- Perform other related duties as assigned.
Qualifications
Education and Experience
Current registration as a pharmacist with the College of Pharmacists of British Columbia. Two (2) years’ recent, relevant patient care experience, ideally in a team‑based primary care setting or equivalent combination of education, training and experience.
