Parkdale Nurse Practitioner - Outreach Focus

- Facilitate substance use treatment and care plans
- Conduct health assessments and diagnostic tests
- Provide referrals to internal and external services
- Lead substance use care protocol development
- Engage in communities of practice for knowledge sharing
- Registration as an RN (Extended Class) with Ontario's nursing board
- 3 to 5 years of relevant nursing experience
- Strong knowledge in harm reduction and mental health care
- Effective communication and problem-solving abilities
- Ability to work in diverse, multi-disciplinary environments
