About the role

The Hospital strives to deliver care that is centred on patients and families. Patient and Family Centred Care (PFCC)

is the provision of care that is respectful of, and responsive to individual patient/family preferences, needs, and values

continuity of care and fosters a learning environment. Applicants are required to have a demonstrated knowledge,

understanding and commitment to this care philosophy.

Reporting to the Implementation Coordinator, Medical Transitions Clinic (MTC) and accountable to the Director,

Acute Medicine, Stroke and Seniors' Program, you will be responsible for providing quality nursing care to medically

complex patients in an outpatient setting. As part of an interprofessional health care team, you will practice within the

Professional Standards of the College of Nurses of Ontario, the philosophy of patient and family centered care,

mission, vision, values and the policies of Thunder Bay Regional Health Sciences Centre.

Applicants must possess the following qualifications

• Current unrestricted registration as a Registered Nurse with the College of Nurses of Ontario
• Recent minimum 5 years experience in the care of medically complex patient populations
• Ability to use the Nursing Process (assessment, planning, implementation, evaluation) in conjunction with her/his

basic and ongoing knowledge to arrive at individualized care planning and delivery

• Experience in collaborating and providing consult to interprofessional staff including Physicians, Nurse

Practitioners and other allied health care team members.

• Identify and recognize abnormal or unexpected patient responses and take appropriate action
• Ability to simultaneously manage multiple nursing interventions
• Strong interpersonal, communication and conflict resolution skills
• Patient navigation skills to coordinate patient care
• Current certification in BCLS
• Strong computer skills eg. Mircrosoft Suite, Meditech.

The following skills are considered to be assets for the position:

• Central Venous Line Management
• Discharge planning
• Experience in teaching and counseling

Salary Range: $41.150 to 58.980
