Part-Time Nurse Practitioner in Primary Care

Join Scarborough Health Network’s Interprofessional Primary Care Team as a part-time Nurse Practitioner. Shape patient experiences through comprehensive care and community wellness initiatives. In this role, you will leverage your clinical judgment and experience to provide patient-centered care while collaborating with a diverse team of healthcare professionals. Your nursing practice will focus on chronic disease management, health promotion, and integrated care for individuals lacking primary care providers, enhancing their health journey. Key Responsibilities:

• Evaluate patient needs with psychosocial assessments
• Implement effective health promotion and self-management strategies
• Coordinate specialist referrals and interprofessional consultations
• Monitor and adjust care plans in collaboration with families
• Engage in quality improvement and impact evaluation activities

Requirements:

• Master of Nursing and NP diploma in Primary Health Care
• Active Extended Class registration with the nursing college
• Minimum 3 years of clinical experience, community focus preferred
• Excellent leadership and interpersonal skills
• Committed to health equity and diversity
