Your commitment to understanding the social determinants of health will be crucial in creating meaningful connections with clients and improving their health outcomes. Key Responsibilities:
- Provide holistic primary care to a diverse clientele
- Conduct assessments and develop treatment plans
- Collaborate with healthcare professionals for integrated care
- Participate in community outreach and health education
- Maintain thorough medical documentation
- Medical Degree with Family Medicine training
- Active CCFP certification and good standing with CPSO
- Minimum two years working in a community health setting
- Experience with marginalized communities
- Current Ontario Driver's license for flexibility in care
