ABOUT THE ROLE

Join the Comprehensive Community Support Team (CCST), a dedicated community-based program supporting individuals living with severe and persistent mental illness, including those impacted by addiction. Our client-centred approach focuses on personalized care delivered through a collaborative team of professionals from diverse backgrounds. As an Registered Practical Nurse on our team, you will play a vital role in providing compassionate, holistic support to help clients achieve their recovery goals.

WHAT YOU BRING

Education/Experience:

• Current unrestricted registration with the Ontario College of Nurses (CNO)
• Practical Nursing Diploma from a recognized educational institute 
• Must meet the qualifications to practice within the full scope of the RPN 
• Basic Life Support Certificate (BLS) and First Aid Certification
• Experience supporting individuals living with serious mental illness, intellectual disabilities, and/or concurrent addiction challenges 

Skills/Abilities:

• Strong skills and knowledge in community assessment, psychosocial rehabilitation, crisis intervention, treatment, and recovery support
• Excellent communication, leadership, critical reasoning and problem-solving skills
• Demonstrated understanding and commitment to people-centered care
• Computer proficiency, including Word and Windows, email communication and electronic documentation
• Excellent time management skills
• Recent physical assessment and pharmacology courses
• Ability to work independently and as part of a team
• Evidence of continuing professional development experience
• Demonstrated regular attendance at work

Assets

• Candidates with past community experience in providing personal and clinical support to clients an asset

Conditions of Employment:

• This position shall be conditional upon a satisfactory Police Vulnerable Sector Check (PVSC)  where indicated, to ensure the absence of relevant criminal convictions.
• Will be required to travel to various work locations and provide own transportation
