The Community Health Representative (CHR) serves as a frontline health advocate, educator, and liaison for The Key First Nation. The CHR supports community members by promoting health and wellness, increasing access to health services, providing health education, and connecting individuals and families with appropriate programs and resources.
Working closely with community members, Elders, families, healthcare providers, and partner organizations, the CHR assists in addressing health priorities through prevention, education, outreach, advocacy, and community engagement. The position plays an important role in supporting holistic wellness by recognizing the physical, emotional, mental, and spiritual well- being of community members. The Community Health Representative contributes to improving health outcomes and strengthening community capacity through culturally safe and community-driven health initiatives.Vacancy posted 10 hours ago
