Sous la supervision de la directrice de l’amélioration de la qualité et de promotion de la santé et en tant que membre de l’équipe multidisciplinaire du Centre de santé communautaire Hamilton/Niagara , l’agent.e en promotion de la santé est responsable de la planification, de la mise en œuvre et de l’évaluation de programmes et d’initiatives visant à améliorer la santé et le bien-être des individus et des communautés. Ce rôle implique la mobilisation communautaire, l’éducation à la santé ainsi que le développement de partenariats afin promouvoir le développement sain et équilibré des communautés francophones des régions de Hamilton et du Niagara.
Responsabilités principales :
- Concevoir, planifier et mettre en œuvre des initiatives de promotion de la santé qui sont pertinentes, adaptées aux besoins de la communauté et visant à améliorer la santé, le bien-être et la qualité de vie des populations, en tenant compte des déterminants sociaux de la santé ;
- Effectuer des recherches sur les pratiques exemplaires dans le domaine et proposer des programmes et services innovants et fondés sur des données probantes ;
- Élaborer et mettre en œuvre des stratégies visant à améliorer la santé et le bien-être des communautés desservies ;
- Préparer et animer des activités pour les groupes ciblés selon les priorités organisationnelles et les exigences des bailleurs de fonds ;
- Établir et maintenir des partenariats efficaces avec des intervenants multidisciplinaires et des organismes communautaires, tant à l’interne (CSCHN) qu’à l’externe, dans une approche collaborative ;
- Participer activement aux initiatives communautaires contribuant à l'amélioration des conditions de vie et à la réduction des inégalités en santé ;
- Favoriser l’engagement communautaire afin de renforcer la résilience des communautés et le bien-être collectif ;
- Concevoir et mettre en œuvre des outils d’évaluation pour mesurer l’impact des activités et des programmes en s'appuyant sur les indicateurs établis par les bailleurs de fonds et le CSCHN ;
- Préparer des rapports administratifs, compiler les statistiques relatives aux activités et assurer un suivi des résultats obtenus ;
- Effectuer toute autre tâche connexe à la demande de la direction du secteur.
Responsabilités connexes :
- Mettre en pratique les valeurs du CSCHN en adoptant une approche de travail qui favorise la promotion de la santé ;
- Protéger sa propre santé et sécurité en respectant la loi et en adoptant les politiques et les méthodes de travail sécuritaires du CSCHN.
Formation/qualifications
- Posséder un diplôme d’études universitaires ou collégiales dans une des disciplines en sciences de la santé, sciences de la vie, promotion de la santé ou en service social, OU posséder une expérience de travail et formation équivalentes.
Aptitudes et expériences:
- Connaissance des problématiques reliées aux déterminants de la santé ;
- Expérience en planification, en promotion de la santé et gestion de programmes, d’activités et de projets ;
- Excellentes compétences en communication, en animation et en relations interpersonnelles.
- Engagement envers les interventions axées sur la collectivité et la connaissance des ressources communautaires ;
- Aptitude à travailler auprès d'une population multiculturelle et au sein d’une équipe multidisciplinaire ;
- Connaissance et habileté à utiliser les logiciels informatiques de base ;
- Excellente maîtrise du français oral et écrit, aisance de communication orale et écrite en anglais ;
- Être disposé.e à travailler selon un horaire variable et à se déplacer pour répondre aux besoins de la clientèle ;
- Posséder un permis de conduire valide et avoir accès à un véhicule automobile.

Health Promoter

Under the supervision of the Director of Quality Improvement and Health Promotion , and as a member of the multidisciplinary team, the Health Promotion Officer is responsible for planning, implementing, and evaluating programs and initiatives aimed at improving the health and well-being of individuals and communities. This role involves community mobilization, health education, and partnership development to promote the healthy and balanced development of Francophone communities in the Hamilton and Niagara regions.
Key Responsibilities
Design, plan, and implement health promotion initiatives that are relevant, community-driven, and responsive to social determinants of health.
Conduct research on best practices and propose innovative, evidence-based programs and services.
Develop and implement strategies to improve the health and well-being of the communities served.
Prepare and facilitate activities for target groups in alignment with organizational priorities and funders’ requirements.
Establish and maintain effective partnerships with multidisciplinary stakeholders and community organizations, both internally (CSCHN) and externally.
Actively participate in community initiatives that contribute to improving living conditions and reducing health inequities.
Foster community engagement to strengthen resilience and collective well-being.
Develop and apply evaluation tools to measure the impact of activities and programs, based on indicators established by funders and the CSCHN.
Prepare administrative reports, compile activity statistics, and ensure follow-up on outcomes achieved.
Perform other related duties as assigned by sector leadership.
Additional Responsibilities
Demonstrate and uphold CSCHN’s values by adopting a work approach that promotes health promotion.
Protect one’s own health and safety by following legislation, as well as CSCHN’s policies and safe work practices.
Education and Qualifications
University or college degree in Health Sciences, Life Sciences, Health Promotion, or Social Work,
or equivalent combination of education and experience.
Skills and Experience
Knowledge of issues related to the social determinants of health.
Experience in planning, implementing, and evaluating health promotion programs, projects, and activities.
Strong communication, facilitation, and interpersonal skills.
Commitment to community-based interventions and familiarity with community resources.
Ability to work with multicultural populations and within a multidisciplinary team.
Proficiency with basic computer software.
Excellent oral and written French skills, with strong oral and written English communication skills.
Flexibility to work variable hours and travel to meet client needs.
Valid driver’s license and access to a vehicle.
