The Role

As a Foundation Pharmacist within Primary Care, you will provide a safe, effective, and patient-centred clinical pharmacy service to a defined patient population, working as part of the multidisciplinary healthcare team under the supervision and direction of a Senior Pharmacist. The post holder will support the safe, appropriate, and cost-effective use of medicines across a range of primary care settings, including GP practices and care homes, while undertaking the NES Foundation Training Framework (or equivalent) to develop the knowledge, skills, and competencies required for autonomous clinical practice.

• 2 x Permanent Roles
• Blair Court, 100 Borron Street, Glasgow, G4 9XE
• 36 Hours (Monday to Friday)

Key Responsibilities

The post holder will provide clinical pharmacy services to patients within primary care settings, ensuring the safe, effective, and cost-efficient use of medicines in line with local and national guidance. Working closely with GPs, nurses, community pharmacists, and other healthcare professionals, you will undertake medication reviews, monitor treatment outcomes, identify and address prescribing issues, and provide expert medicines advice to both healthcare professionals and patients. The role involves supporting medicines reconciliation, responding to prescribing enquiries, promoting formulary compliance, contributing to quality improvement initiatives, and helping reduce medicines waste. You will educate and support patients, carers, and multidisciplinary teams on medicines-related matters, participate in clinical governance activities, monitor medication safety, and contribute to service development projects. In addition, you will maintain professional registration and competence, complete foundation training requirements, and ensure practice is delivered in accordance with current legislation, professional standards, and NHS policies.

Knowledge, training, qualifications and/or experience required to do the job:

• MPharm Degree or equivalent qualification leading to registration as a pharmacist.
• Registered with the General Pharmaceutical Council (GPhC).
• Commitment to completing the NES Foundation Training Framework (or equivalent).
• Knowledge of medicines optimisation principles and safe prescribing practices.
• Understanding of clinical pharmacy practice within primary care and multidisciplinary team settings.
• Knowledge of medicines legislation, governance arrangements, and professional standards.
• Understanding of prescribing systems, medicines reconciliation, and medication review processes.
• Awareness of national and local prescribing guidelines, formularies, and protocols.
• Ability to interpret clinical information and apply evidence-based practice.
• Commitment to continuous professional development and GPhC revalidation requirements.
• Training in clinical monitoring activities, such as blood pressure monitoring and other patient assessment techniques as required.
• Understanding of quality improvement, audit, and service development methodologies.
