Lead Care Coordinator, Residential Programs

POSITION SUMMARY:

Under the direction of The Hearth's Program Supervisor and Safe at Home Society Management Team, the Lead Care Coordinator (LLC) provides case management supports for program participants of Safe at Home Society Supported Housing Programs who may be impacted by mental health and/or substance use concerns, trauma and/or intergenerational trauma, brain injury and other related conditions. The LLC reports directly to the program supervisor and provides coordination of services with other program staff to ensure continuity of care, stabilization, and support that aims to reduce risk of homelessness, promote housing stability, manage crises, and work towards goal-directed pursuits to improve well-being over time. Working within the framework of a person-centered, harm reduction focused, and compassion-oriented approaches, the LLC supports the provision of services, including providing assistance with unit upkeep, relationship building, instrumental activities of daily living, developing support plans, maintaining wellness and connection to community resources. The LLC can provide support in area-specific streams such as the justice system, homelessness sector, and transitions to/from hospital, etc.

ESSENTIAL JOB RESPONSIBILITIES :

1. Provides support to individuals to promote housing stability, harm reduction, and goal-directed activities geared towards well-being, safety/stability, and increasing independence.

2. Routinely monitor resident’s health.

3. Assist in meeting the objectives of the care/ strategic plans.

4. Ensure all file management and reporting functions are maintained in a comprehensive and timely manner.

5. Maintain up-to-date knowledge of: Mental Health Act, Adult Protection Act and related legislation, the structure and personnel of local community support and government agencies, government departments, institutions and other groups relevant to the Lead Care Coordinator Role and activities within the scope of that role in the support of resident needs and care coordination as well as program objectives.

6. Avails self of opportunities for professional development.

7. Works within the Policies and Procedures of Safe at Home Society Yukon.

ESSENTIAL DUTIES

• Carrying out all duties of the Lead Care Coordinator position in a compassionate, responsive, and accountable manner;
• Centre the person-centered, relational, and flexible values of Safe at Home Society in working with clients and other staff members;
• Being present at the scheduled time of the day ready for the performance of duties;
• Being ready emotionally and physically to perform duties;
• Represent program and agency in a professional manner through embracing and fostering the Safe at Home Society culture and values, respectful communication and ethical decision making;
• Provides support to volunteers and students and other agency assigned to the program area; and,
• Substitutes for and assists other agency staff where skills are applicable

Qualifications

• Completion of a 2-year Diploma or Degree in a health or social services discipline. Bachelor’s degree in social work or other related discipline with relevant experience is preferred;
• Minimum two years’ experience in related position;
• Registration, in good standing, with a regulating professional body (e.g. OCSWSSW), or working towards registration (if applicable);
• Reliable vehicle and drivers abstract required as a condition of employment;
• Proficiency with Microsoft Office software required
• Knowledgeable of community agencies and resources

Work Environment

The role requires a blend of office work and field engagements, including visits to partner organizations and attendance at relevant conferences and workshops.
