OPEN TO PUBLIC-Patient Navigator

The Patient Navigator position is integral to enhancing the health and well-being of individuals and families within the Kwanlin Dün First Nation (KDFN). Based out of the Natsékhi Kų̀ Health Centre, this role involves assisting clients in navigating a variety of clinical and community-based health services, focusing on advocacy and service navigation to ensure that their needs are addressed. The Patient Navigator empowers clients to access essential resources and community services through the management and implementation of complex case management services.

This position requires close collaboration with community members, healthcare professionals, and other stakeholders to advocate for clients' rights, overcome obstacles to accessing healthcare and necessary resources, and provide both emotional and practical support. Responsibilities include facilitating safe, accessible, and inclusive healthcare, coordinating comprehensive wellness planning, enhancing the health literacy of clients and their families, and overseeing the delivery of care for clients with complex health and social needs to ensure a well-managed and integrated approach.

Required Qualifications (what we screen/look for on your resume) : An equivalent combination of post-secondary education, combined with significant current/lived experience in the areas identified below, will equally be considered.

• Bachelor of Social Work Degree with a minimum of 2 years of experience in a related position;
• Experience with counseling, psycho-social assessment, crisis intervention and/or motivational interviewing skills;
• Demonstratable knowledge and experience delivering advocacy, navigation to vulnerable persons with complex care needs including those associated with health conditions, aging, disabilities, substance use, mental illness, and concurrent disorder; and
• Experience working in a mental health facility or residential treatment for addictions facilities is an asset.

Knowledge and skills (what we may assess/interview on):

• Strong relationship-building and communication skills , with the ability to establish trust and work effectively with children, youth, families, community members, and partner agencies.
• Knowledge of child development, trauma, addictions, family systems, domestic violence, and child welfare concerns , with the ability to apply this knowledge in a supportive and strengths-based manner.
• Ability to work collaboratively from a culturally safe and strengths-based approach , supporting the unique needs, experiences, and goals of Citizens and families.
• Excellent judgment, confidentiality, and crisis intervention skills , including the ability to de-escalate situations and respond appropriately to sensitive and complex circumstances.
• Strong organizational and time management skills , with the ability to manage competing priorities, maintain accurate records, and respond effectively to urgent situations.
• Knowledge of KDFN programs, services, and community resources , or the ability to quickly learn and navigate supports available through KDFN, Yukon Government, and community agencies.
• Proficiency with Microsoft Office and electronic case management systems , including the ability to maintain documentation and prepare reports.

Knowledge of Yukon First Nations history within formalized education systems, residential schools and on-going intergenerational effects specific to education and social programming is an asset. Familiarity with Kwanlin Dün First Nation history, culture, goals and aspirations is required for success in the role.

Conditions of Employment:

• First Aid & CPR-C (or willingness to obtain within six months of employment)
• Criminal Record Check with Vulnerable Sector Clearance
• Valid Class 5 Drivers License and annually maintain an acceptable Driver’s Abstract
• TB Clearance
• Immunization record
• Naloxone Training (or willingness to obtain within 3 months of employment)

Salary: $76,531.00 - $127,290.90 per annum ($42.05 - $69.94 per hour) Level 7 *plus benefits
