Gestionnaire de cas - Case Manager

RÉSUMÉ DU POSTE
Le gestionnaire de cas (GC) est le point de contact expert offrant des services de soutien en gestion de cas aux patients et aux médecins. Le GC aidera les patients, les médecins et les infirmières dans l’accomplissement de tous les documents spécifiques au programme. Le GC servira de ressource aux médecins qui prescrivent des médicaments et aux patients qui ont besoin d’un soutien continu. Il joue un rôle clé dans la suppression de toute barrière à l’accès au remboursement pour le traitement des patients, garantissant ainsi qu’ils puissent commencer la thérapie aussi rapidement que possible. Il s’épanouit dans un environnement qui met l’accent sur le travail d’équipe et la responsabilité, possède une grande volonté de servir les clients et favorise des expériences positives pour les clients dans un environnement de travail rapide tout en faisant preuve de flexibilité pour s’adapter aux demandes concurrentes à travers le portefeuille selon les besoins.
FONCTIONS ET RESPONSABILITÉS
- S’assurer que le (la) patient(e) a été inscrit(e) au Programme de soutien aux patient(e)s (PSP). Mettre en place les conditions pour un accueil chaleureux dans le programme.
- Fournir une assistance téléphonique directe aux patient(e)s ayant besoin d’aide concernant le remboursement.
- Fournir aux patient(e)s et aux médecins un numéro sans frais direct.
- Développer des relations solides avec les patient(e)s, les soignant(e)s et leur équipe de soins de santé en instaurant la confiance, l’assurance, la sécurité et le réconfort.
- Faciliter l’accès d’un(e) patient(e) à une médication spécifique en offrant des services d’orientation en matière de remboursement.
- Explorer les options de remboursement offertes au (à la) patient(e) et maximiser sa couverture par l’intermédiaire des payeurs privés et publics, tout en réduisant l’assistance financière offerte par le biais du PSP et/ou les dépenses à la charge du (de la) patient(e).
- Représenter le (la) patient(e) auprès des compagnies d’assurance privées ou publiques afin d’optimiser la couverture des médicaments, et aider à remplir les formulaires d’autorisation spéciale (AS) et/ou d’autorisation préalable (AP), le cas échéant.
- Coordonner avec la pharmacie la couverture et la délivrance des médicaments. Résoudre tout problème de remplissage/facturation, selon les besoins.
- Assurer la liaison avec l’équipe du PSP pour optimiser le soutien et la communication avec le (la) patient(e), le cas échéant.
- Maintenir une communication continue et transparente avec l’équipe de soins de santé du (de la) patient(e).
- Assurer une tenue de dossiers rigoureuse pour garantir la continuité des soins et une expérience exceptionnelle pour les client(e)s et les patient(e)s.
- Déceler et relever de manière proactive les lacunes et les possibilités d’amélioration de l’expérience des patient(e)s.
- Gérer les situations difficiles avec les patient(e)s en faisant preuve d’empathie et de patience, grâce à des solutions axées sur le (la) patient(e).
- Collaborer avec les représentants du site pour répondre aux besoins spécifiques des client(e)s.
- Agir à titre de personne-ressource centrale pour la liaison avec les parties prenantes du programme, notamment les médecins, les infirmier(ère)s, les patient(e)s et les cliniques.
- Fournir des mises à jour aux parties prenantes internes et externes, sur demande et selon les besoins.
- Effectuer la saisie de données, répondre aux appels entrants, passer des appels sortants, gérer les logiciels de courriel et de télécopie, et effectuer des tâches administratives générales, notamment l’envoi de télécopies et le classement.
- Assurer une déclaration rapide et conforme des événements indésirables (EI) et des réclamations qualité produit (PQC) conformément aux exigences et directives de pharmacovigilance de Santé Canada, incluant une documentation précise, une escalade appropriée et une collaboration efficace avec les parties prenantes internes et les équipes réglementaires.
- Participer à des activités de formation continue interne et/ou externe.
- Adhérer aux politiques et procédures de Bayshore.
- Participer aux activités de qualité et aux initiatives d’amélioration continue conformément au système de gestion de la qualité de l’entreprise ainsi qu’aux indicateurs de performance spécifiques au projet.
- Participer à des activités proactives en matière de santé et de sécurité tout en exerçant toutes ses fonctions. Informer le (la) superviseur(e) immédiat(e) de tout risque ou de toute préoccupation en matière de santé et de sécurité.
- Préserver la confidentialité des renseignements relatifs aux partenaires pharmaceutiques et à l’entreprise, et n’en discuter qu’avec le personnel approprié de Bayshore.
- Effectuer d’autres tâches selon les besoins.
JOB SUMMARY
The Case Manager (CM) is expert point of contact offering case management support services to patients and physicians. The CM will assist the patients, physicians, and nurses with the completion of all program-specific paperwork. The CM will act as a resource to physicians who are
prescribing medications and patients who need on-going support. They are instrumental in removing any reimbursement access barriers to treatment for patients, thereby ensuring that they can start on therapy as quickly as possible. DUTIES AND RESPONSIBILITIES
- Ensure patient has been enrolled in the patient support program (PSP). Set the tone for a warm welcome into the program.
- Provide direct telephone assistance to patients requiring reimbursement support.
- Provide patients and physicians with direct toll-free number.
- Develop strong relationships with patients, caregivers, and their healthcare team by infusing confidence, trust, security, and relief.
- Facilitate a patient’s access to a particular medication by offering reimbursement
navigation services.
- Investigate reimbursement options available to the patient and maximize the patient’s coverage via private and public payers, while minimizing the financial assistance offered through the PSP and/or out-of-pocket expenses for the patient.
- Represent the patient with private or public insurers to optimize drug coverage and assist with the completion of Special Authorization (SA) and/or Prior Authorization (PA) forms, when applicable.
- Coordinate with pharmacy drug coverage and dispensing. Troubleshoot any filling/billing issues as required
- Liaise with PSP team to ensure optimal patient support and communication, if applicable.
- Maintain ongoing and transparent communication with the patient’s healthcare team.
- Diligent record-keeping to ensure continuous care and exceptional customer and patient experience.
- Uncover and proactively identify gaps and opportunities for improvement in patient experience.
- Through patient-oriented solutions, manage challenging patient situations with empathy and patience.
- Work with the site representatives to support specific customer needs.
- Act as a central resource liaison for program stakeholders, which include physicians, nurses, patients, and clinics.
- Provide updates to internal and external stakeholders as requested and required.
- Perform data entry, answer inbound calls, make outbound calls, manage e-mail and fax software, and perform general administrative functions including faxing and filing.
- Ensure timely and compliant reporting of Adverse Events (AEs) and Product Quality Complaints (PQCs) in accordance with Health Canada pharmacovigilance requirements and guidelines, including accurate documentation, appropriate escalation, and effective collaboration with internal stakeholders and regulatory teams.
- Participate in ongoing internal and/or external continuing education activities.
- Adhere to Bayshore Policies and Procedures.
- Participate in quality activities and continuous improvement initiatives in keeping with the company’s Quality Management System as well as specific performance indicators for the project.
- Participate in proactive Health & Safety activities while performing all duties. Is responsible to notify immediate supervisor of any Health & Safety risks or concerns.
- Maintains confidentiality of pharmaceutical partner(s) and corporate information and discusses same only with appropriate Bayshore personnel.
- Complete other tasks as requested.
QUALIFICATIONS

- Expertise démontrée dans la prestation d’un service à la clientèle exceptionnel et dans l’établissement de relations positives avec les parties prenantes internes et externes
- Maîtrise de la communication verbale et écrite et des compétences interpersonnelles en anglais et en français (pour les provinces/régions ayant des exigences de service en français)
- Une expérience dans le domaine des soins de santé, de l’industrie pharmaceutique ou des assurances serait un atout
- Excellentes compétences en matière de service à la clientèle, de communication, de résolution de problèmes et de relations interpersonnelles requises
- Sensibilité et engagement à utiliser une approche centrée sur le (la) patient(e); capacité à répondre aux besoins d’un(e) patient(e) en matière de soutien émotionnel ou éducatif et de littératie en soins de santé
- Capacité à prendre des décisions en faisant preuve de discernement et à respecter les délais
- Facilité à travailler dans un environnement sans papier
- Souci d’exactitude et de rigueur lors de la saisie de données pour répondre aux exigences de production de rapports
- Sens aigu de l’organisation, souci du détail et capacité à effectuer plusieurs tâches simultanément
- Excellentes compétences en communication écrite et verbale et en relations interpersonnelles
- Compétences informatiques et techniques solides : Excel, Word, Outlook, tablettes électroniques, portails Web/utilisation d’Internet
- Aptitude à travailler dans un environnement de travail calme avec une capacité avérée à travailler de manière autonome au sein d’une équipe dans un environnement de télétravail/virtuel
- Accès Internet haute vitesse établi au bureau à domicile.

QUALIFICATIONS
- Completion of post-secondary education or equivalent.
- Understanding of provincial and private coverage criteria, Special Authorization processes and how to navigate the Canadian drug coverage landscape is an asset
- Demonstrated expertise in delivering exceptional customer service and fostering positive relationships with internal and external stakeholders
- Must be fluent in verbal and written communication and interpersonal skills for both English and French (Applicable for provinces/regions with French language service requirements).
- Experience in the healthcare, pharmaceutical, or insurance industry would be an asset
- Strong customer service, communication, problem-solving and interpersonal skills required.
- Demonstrates sensitivity and commitment to utilizing a patient-centric approach; is adept to responding to a patient’s emotional, educational, and health literacy needs
- Making decisions using sound judgment, and meeting deadlines.
- Ease of working in a paperless environment.
- Mindful of accuracy and thoroughness of data entry for reporting requirements.
- Strong sense of organization, attention to detail, and ability to multi-task.
- Excellent written and verbal communication and interpersonal skills.
- Strong computer and technical skills: Excel, Word, Outlook, tablets, web portals/internet use
- Ability to work in a quiet working environment with proven ability to work autonomously within a remote/virtual team environment
- Established high speed internet access from home office.
