This is an opportunity to help launch a new, community-connected model of primary care from the ground up. As part of a broader effort to expand equitable access to team-based primary care, the clinic will connect residents to comprehensive health services and support Ontario's goals for primary care attachment. The successful candidate will join a growing interdisciplinary team and contribute to a clinic designed to improve access, support primary care attachment, and make a meaningful impact for residents, especially newcomers and others who have traditionally faced barriers to navigating the health system.

About the Population Health Clinic

The Population Health Clinic is a neighborhood-based, interdisciplinary primary care model that brings health and community services together in one accessible location. Developed through a partnership of Community Health Centres, including Flemingdon Health Centre, Access Alliance, and South Riverdale Community Health Centre, the clinic is co-located within New Circles, a registered charity that provides free clothing, settlement support, employment training, and community connections to individuals and families facing barriers. This is designed to create a low-barrier pathway into the health system and build on trusted relationships already established within the community.

Through a collaborative team of physicians, nurse practitioners, registered nurses, allied health professionals, and administrative staff, the clinic will provide integrated, person-centred care that improves access to primary care, advances health equity, and supports better health outcomes for the communities it serves. Position Summary: The Community Health Registered Nurse, as part of the Primary Care team, is expected to work in an interdisciplinary team to deliver high-quality primary health care services for optimal individual and community health outcomes. We are seeking a Community Health Registered Nurse who is committed to client-centered care, and passionate about working collaboratively to address the social determinants of health, to reduce barriers and improve access to comprehensive health care for the residents of East Toronto and North York. Responsibilities include: Direct Client Care:• Provide comprehensive primary care within RN scope (expanded role). • Coordinate care across the Health Centre and external providers. • Provide telephone triage, advice, and follow-up on results and prevention needs. • Deliver client education and health promotion during assessments and groups. • Provide home visits and support for home-bound clients, as needed. Health Promotion and Education:• Work with internal teams to deliver targeted health education and support community development. • Represent the Health Centre in community projects and partner activities, as needed. • Support outreach program development based on community needs. • Contribute to policies and strategies that advance public health and address determinants of health. • Apply evidence-based practice aligned with evolving community needs. Organizational Responsibilities:• Participate in quality improvement, audits, accreditation, committees, and team meetings. • Support annual planning and evaluation of program goals. • Identify priority populations and help implement effective services. • Maintain knowledge of community services and make appropriate referrals. • Teach/supervise learners and support volunteer placements, as needed. • Participate in professional development and staff in-service training. • Perform other duties as assigned.

Skills and Qualifications:

• Registered Nurse registered in good standing with the College of Nurses of Ontario
• Bachelor’s degree in nursing
• Minimum of three years’ experience as a registered nurse within a primary care setting in the following areas preferred:
  • Assessing (and triaging) patients to determine the appropriate nursing and/or medical care required
  • Chronic disease management, with an emphasis on providing care for patients with diabetes, hypertension and asthma
  • Administering medications, vaccinations, and treatments as directed by supervising physicians
  • Monitoring and assessing changes in patients’ symptoms and conditions and providing updates to attending physicians

• Delivering and advising on women’s health screening tests delivered to patients, including cervical cytology tests, breast examinations and contraception
• Providing advice on travel health and administering immunizations to patients before traveling
• Pediatric and/or perinatal experience is considered an asset

• Skilled in assessment, health promotion & education, counseling, and understanding of community development.
• Able to work as a member of an interdisciplinary health care team.
• Non-judgmental/non-confrontational attitude

Working Conditions

• Work may occur across the Population Health Clinic and community partner locations.
• Some evening and/or weekend work may be required based on clinic, outreach, or community engagement needs.
• The role involves direct clinical care, clinical and systems leadership, interdisciplinary collaboration, partnership and community engagement, clinical documentation, and use of standard clinical and office equipment.
• Police clearance, vaccination requirements, and other employment-related screening may apply according to the hiring organization’s policies.
