ABOUT THE ROLE

Join our interprofessional team supporting older adults to regain independence and live safely at home. This program provides short-term, restorative, and rehabilitative care that promotes health, function, and successful transitions back to the community or to alternative living settings when needed.

As a Registered Practical Nurse, you will:

• Deliver restorative nursing care focused on maximizing client independence.

• Collaborate with a team that includes Physicians, Therapists, Social Workers, and Rehabilitation Assistants.

• Support individualized care planning, monitor progress, and contribute to discharge transitions.

• Coordinate with community supports and make referrals to additional providers when required.

WHAT YOU BRING

Education/Experience:

• Current unrestricted registration with the Ontario College of Nurses (CNO)
• Practical Nursing Diploma from a recognized educational institute 
• Must meet the qualifications to practice within the full scope of the RPN
• Basic Life Support Certificate (BLS)

Skills/Abilities:

• Excellent communication, leadership, critical reasoning and problem-solving skills
• Demonstrated understanding and commitment to people-centered care
• Computer proficiency, including Word and Windows, email communication and electronic documentation
• Excellent time management skills
• Recent physical assessment and pharmacology courses
• Ability to work independently and as part of a team
• Evidence of continuing professional development experience
• Demonstrated regular attendance at work

Assets

• Background in Comprehensive Geriatric Assessments and experience working with seniors considered an asset

Conditions of Employment:

• This position shall be conditional upon a satisfactory Police Vulnerable Sector Check (PVSC)  where indicated, to ensure the absence of relevant criminal convictions.
