The City’s Seniors Services and Long-Term Care Division supports Toronto seniors and residents in long-term care homes to have the healthiest, most fulfilling lives possible through exceptional care and services. Vulnerable individuals who reside in the community are supported through adult day programs, supportive housing services, tenancy support and homemakers and nurses services. Reporting to the Manager, Community Programs, the primary function of this position is to supervise the operations of the division's Supportive Housing (SH) and Homemakers and Nurses Services (HMNS) program within a defined geographic area of the City of Toronto.
Major Responsibilities:
- Implements detailed plans and recommends policies/procedures regarding program specific requirements.
- Supervises, motivates and trains assigned staff, ensuring effective teamwork, high standards of work quality and organizational performance, continuous learning and encourages innovation in others.
- Supervises the day-to-day operation of all assigned staff including the scheduling, assigning and reviewing of work. Authorizes and coordinates vacation and overtime requests. Monitors and evaluates staff performance and approves salary increments.
- Provides guidance, support, and training to staff on issue resolution. Facilitates team meetings to promote collaboration and continuous improvement.
- Carries out labour relations responsibilities including hiring of staff, staff investigations, hearing grievances, recommending and issuing discipline up to termination and recommends staffing level changes.
- Provides input into and administers assigned budget, ensuring that expenditures are controlled and maintained within approved budget limitations as it relates to Community Programs.
- Investigates concerns and complaints brought forward by clients and families, including allegations of misconduct. Engages divisional resources; consults with Legal, Ombudsman, Human Rights offices, Behavioral Supports Ontario, mental health experts, contracted agencies, clients and families. Ensures timely investigation, determines outcome, implements corrective action and ensures ongoing communication with the client and family.
- Participates in the planning and development of new programs and sites by analyzing trends, client needs, performing client audits, collecting data, engaging appropriate partners, leading work groups, preparing reports, staff recruitment and training, securing supplies and equipment, and advocating programs at community outreach events.
- Plans, develops and directs the effective delivery of services to clients in programs run by the Unit including managing capacity and responding to service changes by external agencies, ensuring program continuity and ensuring program policies and procedures are implemented and followed.
- Oversees the provision of service to clients, providing guidance to staff managing complex and high-needs cases. Ensures clients are receiving safe and appropriate care, service and individualized support as identified in their care plans.
- Acts as a mediator and advocate for clients and families to resolve concerns related to safety, tenancy, parking, power outages, rent, and program expectations by consulting and collaborating with Property Managers, Divisional and Corporate health and safety advisors, and housing safety advisors.
- Ensures staff are working in compliance with the provisions of all applicable legislations and standards of the Ministry of Health.
- Provides leadership to program committees such as Joint Health and Safety, CABR etc., including member recruitment, development of workplans, and oversight of activities ensuring effective communication between the Division, Management, and direct reports.
- Manages and maintains collaborative partnerships with contracted agencies, new corporations, housing providers such as Toronto Senior Housing Corporation, and community organizations to ensure seamless and efficient client-centered service delivery that is fully integrated across all systems and compliant with evolving legislative frameworks.
- Ensures accurate maintenance of clients' healthcare records in compliance with City policies, applicable legislation, and professional standards.
- Ensures the effective functioning of the admission process and waitlist management of all programs run by the Unit.
- Monitors compliance with contracted service agreements, ensures service standards are met, and gaps are identified. Initiates corrective action.
- Provides input into reports such as the quarterly risk management and quality improvement program reports, Advisory Council and Ontario Health.
- Chairs staff meetings and participates on internal and external committees as required.
- Provides leadership to inform continuous improvement initiatives and align program outcomes with divisional goals for the Unit through monitoring and analyzing service delivery metrics though internal audits, surveys, site visits, and client reviews.
- Engages staff in the development of solutions to close gaps and improve service delivery.
- Designs and facilitates presentations to enhance program awareness and cultivate relationships with community partners.
- Provides on call after hours Supervisory oversight and emergency support for high-risk elderly clients, ensuring timely resolution of client issues, care, and service needs in coordination with external care providers to support client well-being and mitigate risk.
- Participates in the design and execution of community outreach strategies for pilot programs, including collection of data, consultation with partners and service providers, and recommends new partnership opportunities for exploration.
- Represents the Unit at divisional and external community tables contributing to advancing program priorities.
- Ensures program operations and reporting are completed within required timelines and are in alignment with policies and service standards.
Key Qualifications:
- Post-secondary education with a degree in Nursing and/or another professional discipline pertinent to the job function or an equivalent combination of education and experience.
- Considerable experience planning, directing and co-ordinating community-based programs.
- Considerable experience motivating, leading and supervising regulated and unregulated staff in a unionized community setting, and handling routine labour relations.
- Experience managing complex client situations involving high risk seniors and/or vulnerable populations in a community setting.
- Experience working collaboratively with external partners, such as community groups, community service agencies, hospitals, staff, clients and families.
- Highly developed interpersonal, communication (written and verbal), negotiation, conflict resolution skills, with the ability to interact and communicate effectively with all levels of the organization, and to forge relationships with internal and external partners.
- Ability to develop, deliver and evaluate programs with a quality improvement focus.
- Ability to produce a clear, concise and professionally presented report.
- Ability to work independently with minimal supervision, and to organize time and workload effectively with competing priorities and demands.
- Sensitivity and understanding of the diversity of knowledge, languages, literacy levels, culture and ethno-racial background in communities throughout Toronto.
- Ability to function effectively in a service oriented, politically sensitive environment and to balance the needs of the organization and the public.
- Thorough knowledge of relevant government legislation such as the Homemakers and Nurses Services Act, Connecting Care Act, Ontario Welfare Act, Ontario Disability Support Program Act, Accessibility for Ontarians with Disabilities Act, Occupational Health and Safety Act and the Municipal Freedom of Information and Protection of Privacy Act.
- Ability to navigate computer systems, software and applications such as Microsoft Word and Excel.
- Ability to work flexible hours, which may include evenings and weekends depending on operational requirements.
- Ability to work remotely and onsite as required.
- Ability to travel to various sites and/or work locations within the City of Toronto.
