Job Summary:
The Nurse Practitioner (NP) delivers advanced nursing services in accordance with VON and provincial regulatory standards. The NP provides comprehensive, evidence-informed care across the lifespan, including assessment, diagnosis, management, health promotion, and disease prevention. The NP demonstrates leadership, advocates for clients and system improvements, educates diverse audiences, and contributes to research and quality initiatives. The NP practices in a culturally safe, ethical, and client-centered manner, supporting VON’s mission as an integrated community care provider.
Key Responsibilities:
- Acts as the senior clinical official for the Clinic (including any associated satellite sites).
- Performs comprehensive and focused health assessments, synthesize data, and formulate differential and final diagnoses.
- Develops, implements, and evaluates evidence-informed management plans in partnership with clients and families, incorporating their preferences, goals, and cultural context.
- Prescribes and manages pharmacological and non-pharmacological therapies, ensuring safe and appropriate medication reconciliation and stewardship.
- Conducts and interprets screening and diagnostic tests as per regulations.
- Utilizes established infection prevention & control techniques to limit the spread of infection in the clinic and home setting.
- Maintains accurate, timely, and secure documentation in compliance with privacy legislation and organizational standards.
- Refers clients to appropriate levels of providers, support providers and to other disciplines utilizing established guidelines.
- Manages a defined patient panel consistent with ministry guidelines and funder expectations.
- Demonstrates collaborative leadership within interprofessional and intersectoral teams.
- Mentors and supports colleagues, students, and staff to build capacity and excellence.
- Participates in and leads quality improvement, safety, and risk management initiatives.
- Contributes to program development, implementation, and evaluation to enhance client and system outcomes.
- Advocates for clients’ access to resources, culturally safe care, and health equity (e.g., ensure client access to care).
- Encourages clients and their families to take responsibility of their own health by involving them in risk factors and health problem identification, goal setting, and the choices of interventions for disease treatment and prevention, and health promotion.
- Uses community assessment data to identify community needs and resources and develops, implements and evaluates appropriate programming.
- Provides cost-effective, organized, outcome directed care on an ongoing basis.
- Identifies and utilizes tools and resources for work and manages these resources through regular maintenance of equipment etc.
- Addresses systemic barriers, discrimination, and promotes anti-racism and inclusion.
- Responds to social, structural, political, and ecological determinants of health and well-being
- Supports development and implementation of policies and practices that improve health outcomes.
- Develops and delivers education to clients, families, staff, and community partners using adult learning principles and technology.
- Evaluates educational outcomes and adapts material and approach as needed.
- Participates in research, quality improvement, and knowledge translation activities.
- Promotes knowledge translation and integration of research findings into practice, standards, and policies.
- Provides strategic advice and regular updates to the Director NP Clinic Operations, Professional Practice and Education, and NP peers to expand sector understanding of primary care across the province and inform VON’s primary care strategy.
- Engage in continuous professional development and support evidence-based change within the organization.
- Practices autonomously and ethically, adhering to all relevant legislation, regulations, and organizational policies.
- Demonstrates effective time management, planning, and organizing of work activities.
- Delegates care appropriately.
- Ensures confidentiality, privacy, and security of client information.
- Manage conflicts of interest and maintain professional boundaries.
- Promotes the goals and values of VON and their role as an integrated community care provider.
- Promotes a safe and healthy workplace ensuring workplace conduct and activities are in accordance with the provincial Occupational Health and Safety Act and Regulations and compliant with the VON Safety Management System, including all Policies, Safe Work Practices and Procedures.
- Abides by all VON policies and work practices.
- Abides by all confidentiality and protection of personal information policies, regulations and practices and ensures appropriate safeguards are in place within their role.
- Works in collaboration with other staff in a team approach to service delivery.
- Collaborates with clients, families, interprofessional teams, community agencies, and health system partners.
- Collaborates effectively with interdisciplinary team members within a community-based clinical setting.
- Participates in the hiring and orientation and evaluation of clinic staff, if required.
- Serves as a resource to VON staff and external partners.
- Master’s degree in Nursing with a Nurse Practitioner diploma from an accredited program.
- Current registration as a Nurse Practitioner in good standing with the provincial College of Nurses.
- Current certification in Basic Cardiac Life Support (BCLS).
- Minimum 2 years of relevant clinical experience in community-based health care settings.
- Demonstrated commitment to ongoing professional development and evidence-based practice.
- Advanced assessment, diagnostic reasoning, therapeutic intervention, and evaluation skills across the lifespan.
- Demonstrated ability to provide culturally safe care.
- Ability to mentor, educate, and lead quality improvement initiatives.
- Skills in evidence evaluation, research participation, and knowledge translation.
- Critical thinking and clinical reasoning abilities for complex healthcare situations.
- Must have personal vehicle and possess both a current driver's license and proof of vehicle insurance.
- Willingness to travel during the day to support care equity and access to care as required.
- Police check with vulnerable sector screening required.
- Current CPR certification is required.
- Current member of the Nurse Practitioner Association in the province of practice preferred.
- Flexibility to work varied hours including evenings and weekends as required.
- Commitment to trauma-informed and harm-reduction approach to care.
- Must be able to wear Personal Protective Equipment (PPE).
