Assistant Care Manager

The Assistant Care Manager (ACM) is a supporting leadership role at HomeFree, dedicated to one high-needs client who receives continuous, 24/7 care in their home.

You’ll partner with the Lead Care Manager (LCM), who owns the client relationship and is the final decision-maker on the client account. Your role will be to extend the LCM’s reach, carry a meaningful share of the day-to-day, and step in with confidence so that care never lapses.

Role at a glance

This is a part-time (0.5 FTE) position. It isn’t a standard 9–5 role. Care needs in the client’s household arise across days, evenings, and weekends. You’ll share after-hours coverage with the LCM and, in coordination with the LCM, you’ll have flexibility to organize your working hours around the account’s needs, scheduled meetings, in-home responsibilities and after-hours coverage.

You must be a Licensed Practical Nurse (LPN) or Registered Nurse (RN) to qualify for the ACM role. The role combines direct, in-home care with care-team coaching and care coordination. You’ll work closely with the client, their family, the dedicated care team and HomeFree’s Clinical Operations team, while reporting directly to the LCM.

This is a salaried 0.5-FTE position based on 20 hours per week. The starting salary range is $41,600 to $46,800 .

What you will do

Client care and clinical coordination

• Provide direct, concierge-level nursing care through care shifts scheduled according to the client’s needs. This direct involvement will help you to maintain familiarity with the client and provide effective hands-on coaching to the care team.
• Monitor and assess changes in the client’s condition, gather relevant clinical information, and collaborate with the LCM and HomeFree’s Clinical Operations team on appropriate care-plan changes.
• Identify and escalate situations requiring additional clinical expertise.
• Maintain accurate, timely documentation of material observations, actions and follow-up.
• Contribute to care reports for the family and weekly clinical updates for the Clinical Operations team.
• Attend interdisciplinary meetings on the LCM’s behalf, as necessary, and brief the LCM afterwards.

Care-team coaching and development

• Be present in the home regularly to gather feedback, resolve issues, and uphold the quality of care.
• Support, mentor, and coach the dedicated care team for this client, helping foster a supportive and high-performing environment.
• Provide input into the hiring and selection of nurses and caregivers for the client’s care team and help onboard and train new team members.
• Identify emerging performance or wellbeing concerns, provide appropriate support and escalate matters that cannot be resolved through coaching.

Family service and communication

• Serve as a trusted secondary point of contact for the family, providing thoughtful and timely updates and handling sensitive conversations with discretion while maintaining a unified HomeFree approach.

Operations and quality assurance

• Support sufficient staffing with the Lead Care Manager and HomeFree’s Resourcing team.
• Maintain inventories of PPE and other supplies in the home.
• Help track and manage care-related expenses for the client’s account.
• Review account documentation and follow up on incomplete or inconsistent records.
• Track safety and quality concerns through to verified resolution.
• Document after-hours issues, actions taken and outstanding follow-up.
• Maintain visibility into open account issues and deadlines.

Partnership with the LCM

• Carry a meaningful share of delegated account responsibilities and keep the LCM informed without requiring constant direction.
• Own a structured weekly handoff summarizing client status, team performance, staffing, family communications, open risks and decisions required.
• Step in confidently when the LCM is unavailable while escalating material clinical, staffing and family decisions as necessary.
• Maintain a consistent approach so the family and care team experience one unified HomeFree account-leadership team.

What success looks like

• The family trusts the ACM to represent HomeFree.
• The care team follows consistent standards across shifts.
• Changes in condition and quality concerns are identified early and tracked through to verified resolution.
• The LCM receives reliable handoffs and is informed early about emerging risks or unresolved issues.
• Staffing, supplies, documentation and follow-up remain under control.
• Care feels personal and anticipatory rather than administratively managed.
• Care-plan changes and clinical direction are implemented consistently across the team.

What you bring

Required

• Current registration in good standing as an LPN or RN in Alberta
• Valid CPR/BLS certification.
• 3-5 years of relevant nursing, homecare, care-coordination or care management experience.
• Experience coaching, precepting, mentoring, training or supervising nurses or caregivers.
• Willingness and ability to provide direct nursing care in the client’s home.
• Schedule and in-home availability.
• Excellent verbal and written communication.
• Strong organization, prioritization and follow-through.
• Reliable transportation.
• Authorization to work in Canada.

Preferred

• Private-duty or concierge experience.
• High-needs or 24/7 client experience.
• Interdisciplinary-team experience.
• Care-plan and quality-improvement experience.

Schedule and location

• You’ll work 0.5 FTE, with scheduling flexibility subject to client needs and coordination with the LCM.
• You’ll participate in a shared after-hours rotation with the LCM. The rotation will be established in advance and designed to remain proportionate to the ACM’s 0.5-FTE commitment.
• You’ll be expected to work regularly in the client’s home, located in southwest central Calgary. You’ll have the flexibility to complete suitable documentation, reporting and administrative responsibilities remotely.

Compensation, benefits and development
