NURSE PRACTITIONER (NP) -- FAMILY PRACTICE

Ajax Family Practice & Walk-In Clinic is seeking a Nurse Practitioner to work closely with one of our family physicians in a structured, physician-led care model.

The Role

This position focuses on supporting a dedicated family physician by providing care to their rostered patients. You’ll play an important role in follow-ups, chronic disease management, and same-day access, while maintaining strong continuity of care under the physician’s direction.

Responsibilities

• Conduct comprehensive patient assessments, diagnoses, treatment planning, and health education within the RN(EC) scope of practice and in accordance with CNO standards
• Support the ongoing care of patients from an established physician’s roster, helping maintain continuity and strong long-term patient relationships
• Manage routine primary care visits, follow-ups, preventative care, and chronic disease management for patients across the lifespan
• Support same-day and urgent patient needs, including episodic care and physician coverage as needed
• Order, interpret, and follow up on diagnostic tests, screenings, and investigations within scope
• Provide patient counseling and education on treatment plans, medications, preventative care, lifestyle modifications, and community resources
• Deliver compassionate, patient-centered, and culturally responsive care tailored to each individual’s needs and circumstances
• Collaborate closely with the physician and clinic team to coordinate care, ensure appropriate follow-up, and support high-quality patient outcomes
• Maintain accurate and timely clinical documentation within the OSCAR EMR system
• Contribute to a positive, supportive, and team-oriented clinic environment focused on excellent patient care
• Assist with preventative care initiatives, patient outreach, and chronic disease monitoring to help improve overall patient health outcomes

• Physician-owned practice with multiple family physicians

• Two-floor clinic connected to an on-site pharmacy
• Located in Ajax, Ontario

Required

• Active practice registration with the College of Nurses of Ontario
• Bachelor’s in Nursing; Master's Degree in Nurse Practitioner
• Successful completion of the Ontario Primary HealthCare Nurse Practitioner Certificate Program is preferred
• Current Membership in the Nurse Practitioners Association of Ontario and the Registered Nurses Association of Ontario (RNAO)
• Advanced nursing care knowledge related to health assessment, diagnostic, pharmacotherapeutics and invasive and minor surgical procedures required
• Minimum of one (1) year recent primary care nursing experience required
• Excellent patient focus and communication skills
• Experience with EMR is an asset
