PROGRAM COORDINATOR – COMMUNITY CONNECTOR
(Contract Term Temporary, Full-Time – Approximately 25 months)
Located on the traditional, ancestral and unceded lands of the Musqueam, Squamish, and Tsleil-Waututh Nations, West Vancouver is a vibrant community on the North Shore and home to parks, beaches, and to sports, recreation and arts amenities. The West Vancouver Community Services Department has an opening for a contract term temporary full-time Program Coordinator – Community Connector. The Program Coordinator – Community Connector will work with older adults who are referred from Health Care Professionals and community partners to connect with community supports and services through referrals, applications, advocacy, and introductions. Reporting to the Customer and Seniors Services Manager, this position will be responsible for:
Build and maintain networks within community and strengthen relationships within the community-based senior serving sector.
Use an asset-based community development approach to identify and mobilize seniors, and organizations providing and supporting older adults.
Engages and participates in learning opportunities such as in Community of Practices, planning tables or networking events.
Connects, liaises, and establishes partnerships with local health care professionals to create and maintain referral pathways.
Maintains an active caseload of seniors with short-term needs through referrals from health care professionals and community agencies.
Use various assessment tools to determine challenges, needs and risks related to healthy aging and develop routines, structures, and resource referrals to reduce risk of frailty.
Interpret participants’ complex physical requirements and social needs.
Maintains a high level of confidentiality in all matters related to clients and community partners.
Supports seniors to access appropriate range of activities and suitable community resources by developing individual wellness plans.
Refer seniors to community-based services, observes and assesses the participant’s engagement with resources including (example: emotional, psychological, and functional status), and modifies activities to meet the participant’s changing needs.
Effectively collaborates within the Agency’s and community’s Seniors Services to provide multidisciplinary care for the best interest of the senior.
Engages and participates in educational training for seniors on topics such as healthy aging and other relevant topics.
Documents participant’s interactions, wellness plans, reports, and other administrative duties as required.
Participates in evaluation of programs including collection of participant data, reporting at regular intervals, and attending Communities of Practices.
The preferred candidate will possess: a strong knowledge of seniors’ issues and challenges related to healthy aging, health and/or recreational settings; ability to work as a member of a team to provide a high standard of educational and recreational programs to seniors; ability to effectively lead recreation programs to older seniors; understanding of the ActiveNet registration system and computer skills and knowledge of routine office practices and procedures.
The successful candidate has completed: post-secondary education in social service or related Human Services field or a combination of relevant education and experience, or an equivalent combination of education or experience; For details please refer to the class specification which is available for inspection on WestNet or contact Human Resources to request a copy.
WVMEA (Union) – Contract Term Temporary Full-Time
36.97 per hour plus 14% in lieu of benefits
After 3 months of continuous service, Contract Term Temporary employees shall be provided with a one (1) time choice between receiving the same benefit, vacation, statutory holidays and leave provisions as permanent employees or fourteen percent (14%) of regular salary in lieu of all benefits. 1620-26-WVMEA-TEMP-33
