LICENSED PRACTICAL NURSE (LPN), HOSPITAL AT HOME

The salary range for this position is CAD $32.84/Hr. - CAD $41.35/Hr.
Under the guidance of the Resident Care Manager and in close collaboration with a diverse healthcare team the Licensed Practical Nurse (LPN) delivers top-notch nursing care to long-term care residents in addition to providing emotional comfort and support to residents and their families. Education:
Graduation from a recognized program for Practical Nurses or an equivalent combination of education training and experience
Registration with the British Columbia College of Nurses and Midwives as a practicing LPN registrant
Ability to teach and identify patient/resident learning needs
A competitive Salary: $32.84-$41.35 CAD

Provide safe and respectful nursing care adhering to established policies individualized care plans and priorities
Conduct thorough assessments monitor signs/symptoms and communicate with the RN/RPN as needed
Utilize evidence-based resources to stay updated on nursing practices
Address patient/resident and family learning needs provide educational materials and make referrals
Attend care planning meetings provide input and advocate for patients/residents
Collaborate with the team to develop and evaluate individualized care plans
Assist with discharge planning based on patient status and external supports
Operate and clean equipment report malfunctions and handle hazardous materials. Perform clerical duties such as answering phones faxing and maintaining records
