Change Manager - Healthcare

On behalf of our Healthcare Client, Affinity Healthcare Solutions is looking for a Senior Change Manager to join the Provincial Digital Referral Program. The Senior Change Manager will play a pivotal role in this endeavor.
Change Management Plans
~ Prepare organizational and referral-domain change management plans that define the approach, key actions, and timelines required to support a successful transformation.
Identify risks and resistance, and develop and implement mitigation strategies to minimize disruption.
Engagement and Communication Plan and Materials
- Implementation Plans
- Develop implementation plans that consider jurisdictional and environmental factors, as well as end-user readiness, to maximize adoption and sustainability of change.
Training Strategy, Plan, and Materials
~ Support the development of the Training Strategy and Plan, including onboarding and training materials, to equip individuals with the knowledge and skills required to adopt and sustain the change, building confidence and reducing resistance.
University degree, community college diploma, or approved special experience equivalency (Yes / No).
Change Management Experience: Minimum of five (5) years of demonstrated experience as a Lead Change Manager on large, transformational projects (Yes / No).
Large, Complex Project Criteria: Projects that cross multiple organizations, business units, and/or interest holders; Language Proficiency: Written and spoken bilingual proficiency in English and French (Yes / No).
