Nurse Practitioner – Youth Wellness Hub (Part-Time, Permanent)
Employment Type: Permanent, Part-Time
We’re seeking a dynamic and compassionate Nurse Practitioner to support youth aged 12–25 within an integrated health and wellness hub. This role offers a unique opportunity to provide rapid, barrier-free access to primary care for young people navigating mental health, substance use, and wellness challenges.
Deliver comprehensive episodic primary care for youth and young adults.
Perform full assessments, including health history and psychosocial factors.
Prescribe medications and order diagnostic tests within NP scope.
Provide health education and integrate psychotherapeutic approaches as needed.
Participate in program development, quality improvement, and community outreach.
Master’s degree in Nursing (Primary Health Care NP specialization).
Experience working with youth and/or in mental health, addictions, or primary care.
Ability to work hybrid (on-site and virtual) and flexible hours, including evenings.
Flexible scheduling and hybrid work model.
