At Hennick Bridgepoint Hospital, your strategic guidance set the tone for interprofessional patient care, staff development, and clinical excellence within an academic rehabilitative and complex continuing care setting. As a Patient Care Manager (PCM) for our Medical Rehabilitation (4N) and Acquired Brain Injury (ABI) and Neurological Rehab (4S) units, you will build team capacity, implement care standards, and contribute to the unit’s ongoing development. Supported by our collaborative clinical leadership team within a state-of-the-art facility enables the delivery of high-quality care that is elevated by a modern and inspiring workspace. This is a setting where cross-discipline teamwork is the engine that powers our patient- and family-centered care. You will cultivate a team dynamic that balances thoughtful clinical engagement with a personable pace, positioning your team to focus on the meaningful, long-term impact of their care.
Support the critical work of our ABI, Neurological and Medical Rehabilitation teams by facilitating a better experience for patients with acquired brain injuries, complex neurological and other medical conditions.
Are you skilled at solving patient care challenges through structured, data-informed thinking? You approach quality improvement by balancing analysis, input from the team, and operational realities. Whether refining a workflow or evaluating performance data, you focus on practical outcomes that benefit both patients and staff. If you’re someone who builds trust across disciplines, you understand how vital collaboration is to consistent, safe care. In practice, that might mean aligning care plans across disciplines, coordinating input during huddles, or working through differing perspectives on planning. These abilities help you create alignment across teams, turning complexity into coordinated action.
About these teams:
Our 4 South unit helps patients with acquired brain injuries and complex neurological conditions regain their independence and transition back to community living. These conditions may result from motor vehicle accidents, brain tumours, nerve injuries, or chronic conditions such as Multiple Sclerosis (MS) and Parkinson’s disease. Our team values continuous learning and a strong clinical acumen, ensuring that every individual’s journey toward recovery is supported by the best possible care.
Our 4 North Medical Rehabilitative Care team supports patients through some of their most challenging and transformative recoveries. We care for patients with a wide range of medical conditions (such as infections, recent surgeries, deconditioning, frailty, and cognitive impairment) who are working to regain strength, mobility, and functional independence. Many patients have multiple diagnoses and require both medical oversight and rehabilitation-focused support to progress with daily activities. Our team is highly collaborative and focused on helping each patient reach meaningful recovery goals.
Establish and uphold unit goals that reflect the philosophy and standards of Sinai Health, while fostering an environment that supports staff development, clinical excellence, and high-quality patient care
Take a lead role in creating a culture of safety, risk mitigation, quality, and service at the unit level and across the portfolio, to improve practices and service delivery
Manage unit-level budgets and lead resource utilization across the unit and broader program to ensure financial accountability and operational efficiency
Provide leadership in fostering and promoting a collaborative practice environment that is based on mutual respect among all team members
Lead, monitor and evaluate quality improvement and program development initiatives using data-informed methods that support departmental and hospital-wide priorities
Encourage staff participation in quality improvement activities for the unit and preparing quality improvement reports
Support the Hospital’s strong academic mandate for ongoing staff development and training of students from all healthcare disciplines
complete regular employee performance appraisals and support the development of learning plans
Be accountable for the quality of care and patient safety at the unit level by actively maintaining a safe care environment. This includes risk identification, safety audits, staff accountability, and ensuring the appropriate structures, staffing, equipment, and procedures are in place
Establish, promote and maintain good public relations through effective communication within the organization
Foster effective labour relations through knowledgeable interpretation and implementation of collective agreements
Work with other members of the leadership team to support the implementation of evidence based practice and ensuring the dissemination of research findings to inform practice and improve patient outcomes
Participate in the strategic planning process to drive organizational change and optimization of human resources programming and organizational development
Job Type : Temporary Full Time (up to 12 months)
Bachelor’s degree in a Health Sciences or related discipline

  • Master’s Degree in nursing, health or business administration, or health disciplines
  • 3 years of leadership experience in a health care environment
  • Applicants with proven equivalent recent and related training and experience may also be considered

Knowledge of nursing and health care disciplines in clinical services and their scopes of practice
Significant experience in quality improvement; success leading, monitoring and evaluating process/systems to identify and achieve efficiencies
Proficient in using Microsoft Office Suite; advanced understanding of the value of information technology and experience in the implementation and integration of strategies for systems and technologies
Flexible with the ability to work efficiently in a fast-paced, multitasking and dynamic environment
Experience in managing people and Human Resource processes, including formal performance reviews and engaging in staff development initiatives
Program planning and evaluation methodology skills are an asset
Demonstrated satisfactory work performance and attendance history
